Job Description
Deliver high-quality, holistic care to registered patients through face-to-face and remote consultations, telephone triage, home visits, and clinical administration. Assess, diagnose, investigate, and manage a wide range of acute and long-term conditions, providing compassionate, patient-centred care. Prescribe safely and effectively in line with current clinical guidance and best practice. Support patients with complex or multiple health needs, ensuring continuity and coordination of care across the wider healthcare system.
Maintain accurate and timely medical records in line with GMC standards and practice policies. Play an active role in achieving Quality and Outcomes Framework (QOF) targets, Enhanced Services, and other performance indicators. Get involved in practice development through audits, significant event reviews, and clinical governance activities. Work collaboratively with a friendly, multidisciplinary team, contributing to practice meetings, MDT discussions, and safeguarding work for adults and children.
Uphold high standards in infection control, information governance, confidentiality, and health and safety. Commit to ongoing professional development, appraisal, and revalidationyour growth and learning will be fully supported.
