Senior Care Coordinator

NFC Health & Social care have an exciting opportunity to join our expanding team. The senior Care Coordinator will continue to promote the good name of NFC Health & Social Care Ltd,

Responsibilities:

As part of the team you will share responsibility for monitoring and supervising staff and customers, preparing rota, providing a smooth and efficient service. providing and achieving the highest standard of quality care. Promote Dignity in Care, Safeguard Adults and Children and promote good infection control standards.

Location:

NFC Health & Social Care Ltd St Helens

Objectives:



To undertake duties, which are considered, appropriate to the post. Exercise leadership and promote a good relationship between staff, service users and outside agencies. Assess the needs of customers and identify problems, plan solutions and effective individual care plans.

Duties:



Be aware of Inspection procedures undertaken by Care Quality Commission (CQC), Local Authority Contracts department, Clinical commissioning Group( CCG), Local Infection Control Teams.

Contribute to inspections as and when required.

Monitor all Care staff.

On call duties as required evening and weekends on a roster system

Work with Management to promote the company and business growth.

Act within NFC Health & Social Cares Code of practice and in a professional manner always.

Co-ordinate care staff rotas.

Cover staff holidays and sickness

Attend care calls if required

Listen to and Support Staff.

Provide Care to customers when and if required.

Liaise with customers and respond to complaints .

Undertake frequent telephone reviews for new and existing customers.

Staff inductions, supervisions and spot checks and reviews as required.

Monitor standards of care.

Maintain accurate records.

Attend and contribute to staff meetings.

Keep accurate records and log information on the care planner system.

About you



You will have knowledge and experience of working with vulnerable adults in similar settings. Excellent communication and interpersonal skills. Good time management and prioritising skills; able to assess competing demands and prioritise effectively. Motivated to improving services for vulnerable people. Flexible and adaptable to changing environments, processes and procedures. Literate and numerate. Able to maintain accurate records in the required format. Health and social care qualification relevant to the role (i.e. NVQ or equivalent relevant experience. Previous experience of Care Coordinating in a fast paced environment.
