Job Description

The Older Adult Mental Health Hospital at Home teamwork with patients during acute episodes of mental illness or mental disorder to provide an alternative to psychiatric hospital admission. We provide an early referral service for community teams to support patients either in crisis or to prevent a further deterioration of their mental health. OAMH Hospital at Home Team work closely with the Older Adult inpatient wards to ensure they identify patients who can continue their treatment at home and facilitate an early discharge. The service aims to empower and enable patients and their carers to take an active role in their treatment.
The service aims to work together with patients, their families, colleagues, and the wider community for outstanding care, enabling older people to live well and thrive at home.
We will also provide you with resources to develop and progress a long-term career as well as flexibility, continual professional development as well as a wide range of wellbeing and support.
This is full time post, but we would consider applicants looking for part time.
Main duties of the job

  • We aim to provide a response to all referrals within 24 hours to include early referrals from community teams
  • We prevent admission by providing intensive support using evidence based interventions whilst working collaboratively with patients and carers
  • We ensure all potential admissions are considered for the OAMH Hospital at Home service
  • We facilitate early discharge planning with inpatient services which is recovery focused
  • We are invited to attend the weekly rapid reviews for the inpatient ward
  • We work with other professionals and services involved in the patients care to provide a comprehensive service.
  • There is high level of independent work as well as being part of a team
  • We cover the whole spectrum of mental health conditions and have the aim to expand the service further by accepting referrals for patients living with dementia
  • We promote the triangle of care to develop and formulate recovery and crisis plans with patients and their families
  • We complete offer and complete Carer Screens
  • We will liaise with external agencies and Community Mental Health Teams to ensure the best outcomes for the patient whilst supported by the service
  • We conduct Complex assessments during the initial contact with a patient
  • We complete Risk Assessments
  • We delegate and manage the shift through an allocation and board round in the morning
