Job Description

An exciting opportunity has arisen for an enthusiastic, innovative, flexible and well-motivated Senior Community Support Worker to join HomeFirst Carrick team.



The HomeFirst team is part of our Integrated team working in Falmouth, Penryn, Truro and Perranporth, and surrounding areas, supporting the patients in our Carrick Primary Care Network.



It will be an expectation of this post that on occasions, the person works flexibly across other areas of care.



Due to the large geographical and rural area our team covers, being able to drive and having access to a vehicle are essential for this post. Mileage expenses will be paid and pool cars are available although their use cannot be guaranteed. Unfortunately, public transport is not sufficient to get to patient visits on time.



You will be well supported by senior clinicians within the team who will offer regular clinical supervision. Continuing professional development will be encouraged and funded.



The service operates 8-8 over 7 days per week. We offer a flexible approach to rostering to accommodate personal working patterns.

The post holder will provide a versatile and responsive service to ensure that opportunities for patients reablement and rehabilitation are optimised within the community setting. The post holder will be part of an Integrated Care Pathway for patients that involved a range of services to prevent hospital admission and support discharge from hospital.



The post holder will undertake specific clinical and therapeutic support work to aid reablement and rehabilitation of patients, planning and implementing interventions and reporting back on progress and any issues to the clinician.



The post holder will work collaboratively with wider stakeholders to maximise patient care, as well as working closely with the clinical and therapeutic assessors in the team.



The post holder will support the Team Lead with non-clinical duties via delegation including staff supervisions, supporting new members of the team, and clinical audits.

In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.



This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly.
