Job Overview


We are seeking a dedicated and experienced Senior Support Specialist to join our team. This role involves providing high-quality support within care environments, supervising staff, and ensuring the delivery of exceptional care services.

To complete assessments for individuals with high and complex care needs who are returning home from hospital or community services. The post holder will identify all risks, gather essential information for Head Office, and ensure the home environment is fully prepared before the care package begins.

They will support the first 48 hours of each new package to make sure the client, family, and care staff are safe, comfortable, and receiving appropriate support. This role also includes delivering hands-on care and being the local point of contact for staff and clients across Staffordshire.

The ideal candidate will possess strong leadership skills, excellent communication abilities, and a solid understanding of care plans and IT systems utilised in senior care settings. Responsibilities



Initial Assessment & Information Gathering



Complete urgent, same-day assessments for new complex packages, including hospital bedside reviews and home visits. Identify all risks and support needs, including equipment, mobility, medication arrangements, environment safety, and family support. Provide clear and concise assessment summaries and risk findings to Head Office. Ensure all information is shared promptly so that care plans, detailed risk assessments, and staff rotas can be completed by Head Office.

Note:

This role does not write full care plans or risk assessments -- only gathers information and identifies risks.

2. Preparing the Home Before Discharge



Ensure essential equipment is in place before a client returns home (e.g., profiling bed, pressure-relief equipment, mobility aids). Communicate immediately with hospital teams, district nurses, and families to make sure the home is safe and ready. Identify any delays, issues, or missing equipment and escalate to the Operations Manager or Registered Manager.

3. Supporting the First 48 Hours of the Package



Attend or oversee initial visits to ensure the service starts safely and smoothly. Monitor the client's comfort, stability, and immediate support needs. Ensure care staff understand their tasks and can work safely in the home. Identify any rapid changes or concerns and escalate appropriately to the local community teams or Head Office.

4. Delivering High-Quality Complex Care



Provide compassionate, person-centred care for individuals with high support needs. Assist with personal care, positioning, hydration, emotional reassurance, mobility support, and medication prompts. Monitor for changes in wellbeing and report promptly to healthcare professionals when required.

5. On-Call Support



Participate in the on-call rota for the Staffordshire region. Respond to staff sickness, urgent visit cover needs, client concerns, or emergencies. Attend urgent visits if needed to support staff or clients. Escalate concerns according to company procedures.

6. Operational Communication



Maintain daily communication with the Operations Manager and Registered Manager during package start-up. Report any concerns, risks, equipment issues, or staffing challenges. Ensure all findings and observations are accurately logged in the company's digital system.

Person Specification Essential



Experience supporting individuals with high or complex care needs in the community. Strong assessment skills and ability to identify risks quickly. Confident working independently and making informed decisions. Ability to support clients and families sensitively during challenging situations. Full UK driving licence and access to a vehicle. Enhanced DBS check.

Desirable



NVQ Level 3 in Health & Social Care or working towards it. Experience working with rapid hospital discharge pathways. Experience supporting and advising other care staff.
