We have an exciting opportunity for a motivated and experienced

Senior Neuro Occupational Therapist

to join our dynamic

Neuro Early Supported Discharge (ESD) Team

in Southampton. This post offers a fantastic chance to work in an established and forward-thinking multidisciplinary neurorehabilitation service that supports individuals recovering from stroke or other neurological conditions to regain independence and quality of life in their own homes.

As a key member of our multidisciplinary team, you will deliver specialist assessment and rehabilitation interventions for adults with complex neurological presentations, primarily within community and home settings. You will work closely with physiotherapists, speech and language therapists, nurses, clinical psychologists, rehabilitation assistants and administrative colleagues, ensuring a collaborative and patient-centred approach to care.

The ESD Team provides intensive neurorehabilitation to patients who have recently been discharged from hospital or are at risk of admission. Our goal is to maximise functional recovery and enable clients to live fulfilling and independent lives. This is a varied and rewarding role where no two days are the same - you will be helping people make tangible progress every day.

Key Responsibilities



Independently manage a caseload of clients with complex neurological presentations, delivering person-centred assessment, treatment, and rehabilitation programmes. Undertake a range of specialist assessments, including functional, cognitive, postural, mobility, and equipment-based assessments, to support holistic and evidence-based interventions. Plan, implement, and review individualised rehabilitation programmes in partnership with clients, carers, and the wider multidisciplinary team. Provide clinical supervision, support, and guidance to junior staff, rehabilitation assistants, and students. Contribute to multidisciplinary meetings, goal planning sessions, and case discussions to ensure continuity of care. Support the team leader in daily operational management, including caseload allocation and prioritisation, when required. Maintain accurate and timely clinical records using electronic documentation systems, in line with Trust policies and professional standards. Actively participate in audit, research, and service development to improve the quality, safety, and effectiveness of patient care. Promote patient and carer education to facilitate self-management and long-term recovery. Work flexibly across community and clinic settings, demonstrating adaptability to changing environments and patient needs.

About You



You will be a confident and compassionate occupational therapist with a genuine interest in neurorehabilitation. You will thrive in a collaborative, multidisciplinary environment and bring strong problem-solving and communication skills to every aspect of your work.

Essential Criteria:



Degree or Diploma in Occupational Therapy with current HCPC registration. Demonstrable experience of assessing and treating patients with complex neurological or stroke-related needs. Experience working within a multidisciplinary team, ideally across both acute and community settings. Excellent communication and interpersonal skills, including the ability to explain complex information to clients and carers. Strong organisational skills and the ability to prioritise and manage a varied caseload. Understanding of clinical governance, risk management, and evidence-based practice. Ability to work independently within the community, often as a lone practitioner, with access to supervision and team support. Access to transport for daily community visits and willingness to travel across Southampton and West Hampshire.

Desirable Criteria:



Post-registration training or specialist courses in areas such as stroke rehabilitation, long-term conditions, postural management, or continence care. Experience in supervising junior staff, students, or assistants. Participation in audit or service improvement initiatives. Involvement in professional networks, special interest groups, or clinical forums.

About the Neuro ESD Service

Neuro Early Supported Discharge Team

is an integral part of the wider

Neuro Rehabilitation Services

in Southampton, which includes the Snowdon Neuro Rehab Unit, Community Neuro Rehab Team, Vocational Rehabilitation Service, Neuro Gym, Transition Team, Community Psychology and Speech and Language Therapy services.
