Our @home service is at the forefront of healthcare delivery and we are looking for a dynamic nurse to join our team. The team brings acute clinical care usually provided in hospitals to patient homes.The service operates 7 days per week, from 08.00hrs to 23.00hrs. Patients using the service will range from those requiring a single episode of care e.g. intravenous therapy for rehydration. To patients who periodically require short term treatment for acute decompensation of chronic conditions. E.g. heart failure.This role will be based within the community visiting patients in their own homes. Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care and requesting of agreed diagnostic procedures/clinical investigations. You will be responsible for the correct administration of prescribed medication including intravenous therapy. Be responsible for assessing and recognising emergencies in the speciality. You will also be able to interpret information and take appropriate action and lead others to do the same.You will be expected to act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines. To be pivotal in encouraging and managing appropriate referrals, attending ward rounds, MDT and other meetings. Ensuring that patients receive high quality clinical care and a good patient experience, having regard for their social circumstances, customs, religious beliefs and doctrines.
