The Role

An opportunity has arisen for a Senior Occupational Therapist within the Discharge to Assess function of The Independent Living Service. The Independent Living Service has 26 staff made up of Occupational Therapists, Disability Officer and Community Assessment Officers and includes the Sensory Team and Telecare Team. The aim of the post is to aid with discharge facilitation, rehabilitation and community follow up. The posts will sit alongside the Home Support Reablement Service and looks to assist people upon discharge who could be at possible risk of readmission to hospital without support.

This post offers an opportunity to contribute to the ongoing development of innovative ways of working across health and social care in order to improve the experience of service users and carers and to promote wellbeing and independence.

The ability to develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in inter-disciplinary team working.

The OT role will involve undertaking assessments/reviews of need under the Care Act 2014, assist with advice /techniques, equipment, adaptations and provision of support as necessary and also link people in to universal and voluntary services. They will also assist colleagues within other areas of the service as necessary.

The Senior OT will work within the Discharge to Assess at Home function of the Independent Living Service and sit alongside Home Support Reablement, Community Therapy and the Intermediate Tier Social Work team. This role requires the person to work weekends and bank holidays on a rotational basis to support the delivery of the service 365 days of the year.
