Mersey Care Foundation Trust is delighted to recruit a Senior Personality Disorder Case Manager to work in our Personality Disorder (PD) Hub Service based in Waterloo in Liverpool. The present recruitment position will be working in the Warrington and Halton area, during an expansion of the well-established and award winning services in the Liverpool and Sefton footprints of the trust.



The successful applicant will case manage up to 10 complex service users accessing local secondary care mental health services with a diagnosis of Personality Disorder, predominantly Emotionally Unstable Personality Disorder (EUPD). Initially the caseload will also encompass service users who may have been in long-term out-of-area placements (rehabilitation/low secure).



The model of care being delivered is intensive case management that involves encouraging problem solving skills and self responsibility towards recovery amongst the service users, alongside the coordination of care with internal and external agencies. The predominant model of intervention will be Structured Clinical Management (SCM) alongside the application of MCFT Borderline Personality Disorder Policy.



It is essential that successful candidates have significant understanding of SCM. Successful candidates will also engage in the psychological, social therapeutic and trauma based intervention with the wider PD Hub population.



The role will work in liaise with Core 24 services (e.g. AE Mental Health Liaison) to realise the most appropriate outcomes for service users under the care of PD HUB Services within such contexts.



Beyond the care coordination of a caseload of complex personality disordered service users successful candidates will:



Engage in the delivery of personality disorder approved NICE therapies (e.g. SMC, MBT, DBT individual and group work), trauma based psychotherapeutic interventions, (e.g. Sensory Motor, EMDR).



Lead social therapy and therapeutic community interventions within the PD Hub Service (Day and Safe Service).



Lead in the consultation and training of other teams within local community based services (internal and external to MCFT).



Supervision, mentorship and training of junior staff.

To function as an associate nurse/named nurse/care coordinator to service users assisting in assessment, planning and evaluation and review of care, within a recovery focused model of care

Principal Responsibilities:

Clinical



To engage with service users using the PD Hub in a positive and non-defensive manner, especially during crises when calmness and hope need to be maintained.



To coordinate the care of service users carrying out assessment planning, implementation and evaluation of their care under the guidelines of the care programme approach to ensure that the service user's care plan is developed to meet the assessed need of the individual.



To liaise with Core 24 services, e.g. AE Mental Health Liaison to appropriate outcomes for service users under the care of PD HUB Services.



To complete complex assessments, psychological formulations and care planning in the form of Extended Care Planning with reference to Trust's BPD Policy.



To participate in psychological interventions for personality disorder (Structured Clinical Management and PD and trauma therapies) under supervision from senior clinicians.



To lead the process of consultations, training and support to other community and inpatient services in regard to personality disorder.



To lead in the linking and engagement of service users to the other PD Hub Services (PD Day/Safe Services).



To ensure a gradual and planned transition of service users at the end of their period of care under PD Case Management Team (e.g. supported follow up through discharge support groups).



To liaise with family members, carers and significant others when completing the assessment and developing a care plan.



To react pro-actively in cases of service user crisis and put in place immediate plans to resolve the situation with supervision and guidance as appropriate. To undertake and supervise risk assessments and formulation of risk management plans for service users under your care, taking note of the contributions from other members of the team, family members carers and significant others.



To ensure Safeguarding Children and Adults requirements are followed and participate in meetings as appropriate.



To direct unqualified staff in order to ensure delivery of a high standard of care to service users.



To participate in setting standards for care based on sound evidence based and research findings.



To participate in the systematic monitoring and evaluation of service users, including the use of outcome measures.



To communicate to other members of the multi-disciplinary team information which has an impact on the service user's care.



To use verbal and non-verbal communication skills gained through training or experience to impart sensitive information.



To check understanding of information where there are barriers to effective communications.



To give and receive accurate up-to-date information regarding service users' care in reviews and multi-disciplinary meetings etc. These reviews may be external to the trust.



To have accurate and up-to-date knowledge of the Mental Health Act / Mental Capacity Act and other relevant legislation.



Where appropriate to job role take responsibility for the safe administration of medication to service users ensuring that the Trust policies and procedures and the Nursing & Midwifery Councils regulations are adhered to.



To ensure that all relevant information with regards to a service user care and treatment is documented accurately and legibly into the health record. To supervise and monitor the entries made by unqualified staff.



Provide reports both in written and verbal formats as requested.



To ensure that all incidents or near misses are documented as per trust policy and that any follow up action is taken.



To react pro-actively in cases of service user crisis and put in place immediate plans to resolve the situation with supervision and guidance as appropriate.



To supervise nursing, social work, AMHP and other students.



To actively lead/participate in team meetings and team based continuing professional development.



To participate in the induction of new staff.



To actively participate in team meetings in order to positively influence positive team working and the development of research-based practice.
