Senior Practice Nurse (lead Nurse)

Job Description

We are looking to recruit a full or part-time Lead Practice Nurse with an excellent all round knowledge of chronic diseases and has an interest in Sexual Health, to join our nursing team of 6 who provide excellent patient care to over 12,000 patients. This role would suit someone that has a positive can do attitude with skills to develop and support the team. You will be an actively participating member of the Primary Health Care Team, caring for practice patients including treatment, preventative care, screening, health education and chronic disease management, according to practice protocols. Ensuring patient confidentiality in all times.

MAIN RESPONSIBILITIES:



1. Assess, plan, provide and evaluate nursing care to meet the care needs of individuals and groups in the practice population, managing own caseload.

2. Manage care programme for patients with acute and chronic diseases including groups such as; Diabetes, Women's Health, CHD, Asthma, COPD and Hypertension.

3. To provide general nursing care on the assessment and management of wound care and general health problems. This includes support of HCA/phlebotomist role with blood tests, ECGs, blood pressure checks, 24 hour blood pressure monitoring, stock control.

4. To contribute to the development and review of practice protocols.

5. To, support specific public health/health care projects, eg. Flu campaign, Hib vaccination, Smoking Cessation.

6. To work as part of the practice nursing team to ensure that the whole population within our area is given the highest level of care.

7. Support practice development plans, other local and national initiatives and the GMS contract overall.

8. To contribute to research and audit as required

9. To provide holiday cover for other practice nurses as required and help with additional clinics when the demand is great, such as Covid/Flu clinics ( some weekend work)

10. To attend staff meetings as requested.

11. To undertake any other duties within the professional and general competence of the postholder, as required by the Lead Practice Nurse, the Practice Manager or clinical lead GP.

12. Other nursing functions such as wound care, childhood immunisations, ECGs, cervical cytology, wound management

Administration and professional responsibilities: -



Participate in the administrative and professional responsibilities of the practice team. Ensure accurate completion of all necessary documentation associated with patient health care. Ensure collection and maintenance of statistical information required under the GP contract/QOF. In conjunction with other practice nurses audit all areas of work for effectiveness and future planning.
