Be integral in supporting patient safety and driving high quality, evidence-based care across the care team. Work as a shift lead, ensuring appropriate allocation of patients, providing support to junior staff where needed. Be a role model to the team, demonstrating a high level of professional behaviours, clinical skills and professional curiosity and management of risk. Support the professional development of the clinical team, including teaching, providing feedback and fostering professional growth.
Work with the senior management team to support effective change management. Participate in quality governance processes including audit and contributing to workstreams such as safeguarding, transition, medicines management. Support engagement with colleagues within and beyond the hospice. Participate in the delivery of high-quality nursing care to children and young people who receive respite and end of life care at the hospice.
Provide clinical nursing assessment, robust care planning and evaluation of care. Participate in the training and development opportunities and frameworks provided by the hospice education team. Use clinical judgment to manage the deteriorating patient, following care plans, escalation of care and the use of patient safety tools. Work within your scope of practice and clinical competence, asking for help from managers or external professionals when appropriate.
Advocate for the child and young person and their families, supporting and promoting decisions, choices and best interests. Work as part of a team, providing support and supervision to students and junior members of staff. Ensure the holistic care of children and young people, by liaising with and referring to the wider support teams within the hospice Provide end of life care to children or young people staying at the hospice. Provide assessment of children who are end of life ensuring symptoms are controlled, liaising with the specialist palliative care teams as required.
Ensure exemplary communication skills and participate in the MDT planning of childrens care. Maintain accurate contemporaneous patients records as per hospice policy and NMC framework. Follow all hospice safeguarding policies and procedures ensuring escalation of safeguarding concerns in a timely manner. Engage in clinical supervision to support own personal and professional development and engage in the facilitation of this for registered and non-registered members of the care team.
Work as part of a rota to meet the 24/7 needs of the service. Occasionally work as part of an on call. Work within the code of the NMC at all times. Support and drive the team to ensure the strategic objectives of the hospice and care strategy are delivered.
Demonstrate a sound understanding of clinical risk and ensure appropriate mitigations are in place. Support young people as they transition to adult services. Undertake driver and poolside training to enable children and young people to experience the full range of hospice activities.
