The role
As we continue to evolve our services to meet the needs of our community, we are looking for a
Senior Staff Nurse
to join our
Hospice at Home
team.
The role will provide a responsive, flexible and high-quality community specialist palliative care service to patients and those important to them.
You will:
Provide specialist nursing care to patients in the last weeks/days of life.
Complete assessments of patients referred to the Hospice at Home service at any stage in their palliative diagnosis.
Transfer patients care to other hospice services as appropriate when nursing interventions are no longer required.
Work independently during weekends and as part of a
7-day rota
, including bank holidays.
Lead and coordinate care planning, ensuring safe, effective, and personalised care.
Delegate appropriately and support the work of
Senior Healthcare Assistants (SHCAs)
.
Collaborate closely with the wider
multi-disciplinary team (MDT)
.

About you
We are seeking a
highly skilled and experienced nurse
who is:
Ideally experienced in
community nursing and/or palliative care
.
Confident working independently, including lone working at weekends.
Committed to
developing professionally
within a supportive and values-led environment.
Skilled at
delegating tasks
and coordinating care across a team.
A strong communicator with the ability to work collaboratively with an MDT.
Driving licence and access to a vehicle are essential for work purposes.
