ROLE SUMMARY:



To support the Service Coordinator in the delivery of high quality supported living services in line with company policy to enable people to live independently and safely within their own home, living the best lives possible.

MAIN DUTIES / RESPONSIBILITIES:



To observe and monitor the day to day running of services in area of responsibility in close liaison with the Service Coordinator and escalate issues as required.

To monitor and escalate routine service and staffing issues, offering appropriate advice and support to peers, ensuring the smooth and effective running of Living our Lives locations in area of responsibility

To act as a role model for staff and teams within area of responsibility

To attend and contribute to staff meetings.

To be aware of responsibility to safeguard vulnerable people and promptly report concerns of abuse.

To ensure that daily records are updated and that care plans are accurate in accordance with the service user's needs.

To be pro-active in supporting service users to engage with community activities, diversionary activities and therapeutic interventions to improve their health and well-being and reduce social isolation.

To provide support to service users as identified within their person-centred support plan.

To implement the principles of the Mental Capacity Act and support people to live a life of their choice.

To undertake domestic and cleaning duties in the home whilst supporting service users' independence, as identified within their person-centred support plan.

To support service users with all activities of daily living and personal care including personal hygiene needs, nutrition, mobility and any other activity identified.

To administer or prompt medication according to the service users support plan and ensure that the organisation's medication procedures are accurately followed.

To undertake and support service users with shopping and cooking.

To promote equality, diversity, rights and responsibilities.

To drive the promotion of health and social wellbeing and assist people to maintain a sense of identity - this includes promoting and maximising social and developmental opportunities.

To monitor health and safety of themselves, other workers and Individuals' and observe health and safety requirements relevant to activities, escalating any concerns appropriately and promptly.

To complete weekly financial and medication audits under the direction of the Service Coordinator
