Main Duties and Responsibilities
First Line Management
In addition to providing direct contact with clients and maintaining the role of a Support Worker, the Senior Support Worker will provide first line management to a small team.
Duties include:
Leading, coaching and developing the team to provide high quality, personalised care
Developing, maintaining and reviewing service and individual's care plans and outcomes
Providing supervision to team members and associated duties (e.g. identifying training needs, performance management support, reviewing holiday requests, etc.)
Supporting service co-ordination for new and existing clients and their workers
Working closely with other Senior Support Workers to meet the needs of the service and respective teams
Liaise with Care at Home Manager and other staff within the organisation to support the organisational goals
Assessing and developing client care plans and reviews, ensuring information is accurate, up-to-date, and documentation and related processes are adhered to
Provide on call assistance
Personal care of service user
To assist the service user with personal care needs, including: washing, bathing, showering, dressing, oral hygiene and toilet needs. Other duties can include:
Getting up and / or assisting service user to/from bed
Moving and assisting with or without the use of aids and adaptations in accordance with the service users risk assessment and health and safety requirements
Dressing and undressing
Assisting/support with toilet needs including changing of incontinence pads, catheter and stoma care
Assisting/supporting activities of daily living
Awareness of health needs
In an emergency, recognise the need to alert the carer / emergency services
Co-operation with relevant health and social care providers including GP, district nurse, health visitor, social worker etc
Prompt or remind to take medication and complete relevant medication paperwork
Food and nutrition
Preparation of food, snacks and drinks
Dealing with dietary needs, including health related needs (e.g. diabetes, religious preferences, etc.)
Support/assisting with feeding or drinking
Following feeding protocol as assessed by relevant health professionals
Managing and promoting good food hygiene
Assistance with domestic tasks
General tidying
Washing up
Laundry/ironing
Dealing with household refuse
General domestic cleaning as agreed at time of assessment
Where appropriate, tasks agreed to deal with pets
Assistive technology - community alarm and telecare
To promote the use and maintenance of community alarms and telecare that help support carers and service users
To encourage service users to wear pendants/wrist-straps connected to the community alarm service
To report to the carer any identified faults or issues with the use of telecare, including community alarms
Social and emotional Support
Provide companionship
To provide social and emotional support
To accompany on outings
To promote other carers centre services and signpost to other relevant support services
Accompany service users to social events and appointments when required
Lone working
To be able to work on your own within the community
To liaise regularly with the Centre regarding any information relevant to the carer or the service user
