In this position, you'll not only provide hands-on care and support to adults & older children with learning disabilities and mental health needs in a welcoming supported living environment but also take the lead in raising the standard of care. Key Responsibilities:



House Management:

Responsible for arranging activities together with service users and updating their weekly schedules to accommodate these. Managing day-to-day responsibilities and ensuring the team are supported enough to achieve desired outcomes. Ensuring petty cash is adequate and supplemented where and when necessary. Holding weekly meetings with house to make meal plan and ensure shopping is completed on a weekly basis

Service User Management:

Have weekly 'check-ins' with service users and review the service they are receiving. Collaborate with the Supported Living Manager to assess service users' needs, create care plans, and conduct periodic reviews. Assist in the development of comprehensive care plans and contribute to their implementation.

Medication Management:

Oversee the medication administration process, ensuring accuracy and compliance with prescribed regimens. Train and support other staff members in medication management procedures. Audit eMar and report any non compliance.

Emotional Support:

Act as a role model for providing emotional and psychological support to service users. Mentor and guide support workers in fostering meaningful connections and social interaction among service users.

Assessment and Planning:

Plan person centred team meetings in order for the team to be supported with the implementation of care and support for specific individuals. Assess training needs of support workers and provide more support as and when needed.

Household Support:

Lead efforts to maintain a clean and organized living environment, delegate tasks to other support workers, and ensure all daily living activities run smoothly.

Safety and Security:

Take the lead in identifying potential hazards and ensuring the safety and security of service users. Train and guide support workers in following established safety protocols.

Behavioural Support:

Demonstrate expertise in implementing behaviour management strategies and crisis intervention techniques. Train and guide other staff members in responding effectively to service users challenging behavioUrs.

Documentation:

Maintain comprehensive and accurate records of care provided and service users' progress. Review documentation from support workers to ensure completeness and accuracy.

Personal Care:

Provide exemplary personal care support to service users, ensuring their dignity and respect. Assist and guide other support workers in maintaining high standards of personal care.

Communication:

Serve as a primary point of contact for communicating with service users, families, and healthcare professionals. Ensure that all relevant parties are informed of developments and changes in service users' care and well-being.

Professional Development:

Stay at the forefront of best practices in supporting individuals with learning disabilities and mental health illnesses. Lead training and development sessions for support workers and complete monthly supervisions for your team. Ensure all support workers are signed up to Social Care Wales and relevant NVQs and provide support where necessary.

On Call:

There will be an element of 'On Call' duties. Being available to aid and advise support workers as and when needed. This will be undertaken during your '4 days on'. Assist to cover any absence and make any necessary changes to ensure adequate cover.

Qualifications:



High school diploma or equivalent (additional relevant education/training is a plus). QCF/NVQ level 3 or working towards this. (Level 2 will be considered) Significant experience in a care or support role, particularly with individuals with learning disabilities and mental health illnesses. Strong understanding of person-centred care and a proven commitment to promoting residents' independence. Exceptional compassion, empathy, and relationship-building skills. Outstanding communication abilities and a strong sense of teamwork. Patience, professionalism, and the ability to handle challenging situations effectively. Willingness to work flexible hours, including evenings and weekends with elements of On-Call.
