The Role



No two days as a Senior Team Leader are ever the same. You will be the outreach support for the office and the care staff alike. Supporting and ensuring clients are in receipt of premium care services, whilst guiding and supporting staff throughout their care journey with Cream Home Care. This role is designed to support the Care Management team and the business by completing some of the tasks outlined below:

- Emergency cover for clients when required (around your agreed working hours)

- Build and maintain positive relationships with potential and existing clients, their families and other professionals involved in their care

- Completing Client Care Consultations and initial assessments

- Carrying out re-assessments of clients' needs

- Devising person- centred/ outcome- based care plans and risk assessments

- Complete back ground information briefings with staff prior to them visiting a client

- Completing Quality Assurance tasks- such as regular quality assurance and service review monitoring appointments

- Attend meetings with multi-professionals to support our clients

- Complete support visits with care staff to allow for support and guidance opportunities

- Complete Care staff competency assessments

- Be the point of contact for care staff out in the field for any support

- Carry out any other duties deemed necessary for the successful operation of the business as directed by line management.

We provide care to our clients from 6am in the morning until 11pm at night, this is 7 days a week and all year round. We aim to be as flexible and accommodating as possible. We have various shift patterns and options available, with the aim of finding something suitable to meet your work life/ home life requirements. A minimum of working every other weekend is required.

The Fit:



At least 2 years' experience supporting adults in the social care sector Ideally 6 months experience in a senior position within the social care sector- Although not essential Available to work every other weekend The ability to work as part of a team, motivated and driven to be the best A passion to deliver great care with total dignity Great spoken English Two references, one of which must be in social care The right to work in the UK Full UK driving licence and access to your own car
We are heavily invested in technology and making life easier for our Care staff. All of our Care plans are electronic along with medication records and clients notes- No more having to write them lengthy visit notes at the end of the visit- You can now dictate these into the system using the phone which we provide you with.
