To assist service users to live their best lives by assisting them - directly and through others - to exercise choice in order to: improve their mental health and wellbeing; reduce loneliness; increase social participation; work towards goals; develop their independence.

Key Responsibilities

Functional Responsibilities



1. Line manage one or more CSWs and/or volunteer staff

2. Support the management team in the delivery of the service or contract

3. Contribute to team resilience by ensuring appropriate boundaries, escalating issues when necessary.

4. Establish and maintain relationships of trust with service users including helping service users to set, prioritise and achieve goals

5. Work flexibly with service users in both their homes and in the community

6. Value the lived experience of service users and enable them to use that experience to support each other and improve their experience

7. Carry out assessments and determine urgency of service user needs

8. Work with professionals from other agencies and make onward referrals

9. Take a lead on attending external meetings and attendance at multi-disciplinary team meetings

10. Carry out risk assessments and determine urgency of service user need

11. Assist service users to navigate the benefits and education systems

12. Assist service users to manage friendships and family relationships.

13. Accompany service users to appointments.

14. Make timely records which are concise, accurate, written for the service user to understand, using Together computer-based systems.

15. Take a lead on client data management.

16. Write clear word processed and handwritten reports and records in English.

17. Contribute to contract compliance reporting and commissioning reporting.

18. Participate in mandatory and job-related courses and learning activities
