We have an exciting opportunity here at Care & Repair North-East Wales, based in Shotton. Come and join our friendly, dedicated team as a Sensory/Cognitive Impairment Caseworker for Flintshire and Wrexham. You will be responsible for working directly with vulnerable older people with sensory loss and/or cognitive impairments such as dementia and/or stroke to enable them to remain living safely at home. You will develop positive working relationships with health and social care professionals, undertake person-centred assessments, provide advice and tailor support/adaptations to meet the needs of everyone referred to the Managing Better Service.

The principal focus of the service is to assess for essential home repairs, aids and adaptations that support client independence and safety, identify suitable technology and access to other appropriate services. Key to the service is identifying with community, housing and health and social care partners those in critical need, ensuring effective service access for clients and tailoring individual interventions, accessing various grants and financial support that will help address individual client needs.

The purpose of the post is to work proactively with frail older people with sensory loss and/or cognitive impairments to improve their ability to live independently, for as long as possible in their own homes. This will be achieved by working in partnership with community, housing and health and social care colleagues and identifying older people in priority need, supporting individual clients with a person-centred approach in which their specific needs are considered, advocated for and responded to. Specialist training will be given to help you flourish in the role.
