We are looking for Service Coordinator (senior care worker) who is a caring and committed individual who want to work with an organisation that understands how important it is to support individuals with complex health needs.

Many of our service users have complex and challenging needs. Blossom Day Opportunities support workers will work in partnership with health care professionals such as Nurses, Doctors, Therapists and other experts, to ensure we maximise our services users' Health and Social care needs. We also work very closely with our colleagues throughout the Blossom Wellbeing Group.

Blossom Day Opportunities will ensure we support our service users to be involved in the wider community and assist them to engage in activities as required.

Job Summary:

To provide excellent care ensuring you value the needs of others.

To maintain and develop high standards of care for our service users while being aware of their individual needs.

To contribute towards effective communication, by reporting to management team any information given by service users, relatives and / or advocates, incident or observation which may affect service users in any way.

To contribute to the maintenance of a safe and comfortable environment in accordance with health and safety standards.

To monitor the following care standards ensuring the meet CQC guidelines

Assisting with the monitoring of care staff

Assisting with the induction of new staff

Assisting with the implementation and reviewing of care plans and risk assessments

To be a link between the organisation management and staff/service users.

Have held a senior position or equivalent in a care setting

Hold a level 2 qualification in health and social care and be willing to undertake level 3 training within 12 months.

Job Types: Full-time, Permanent

Pay: 13.60 per hour

Expected hours: 36 per week
