We are seeking a dedicated and experienced Care Manager to oversee the delivery of high-quality care within assisted living, nursing home, or care home settings. The successful candidate will be responsible for managing care plans, supervising staff, and ensuring the well-being of residents, particularly those requiring specialised dementia or Alzheimer's care. Duties
Develop, implement, and regularly review personalised care plans tailored to individual resident needs, including dementia and Alzheimer's care.
Oversee daily operations within the care environment, ensuring compliance with health and safety regulations.
Supervise and support care staff, providing leadership and guidance to maintain high standards of service delivery.
Administer medications in accordance with prescribed protocols, maintaining accurate records.
Coordinate with healthcare professionals to ensure residents receive appropriate medical attention and support.
Monitor residents' conditions closely, responding promptly to any changes or concerns.
Organise training sessions for staff on topics such as medication administration, dementia care, and person-centred approaches.
Foster a warm, respectful atmosphere that promotes independence and dignity for all residents.
Manage administrative tasks related to staffing schedules, documentation, and compliance reporting.
Skills
Proven experience in assisted living or nursing home environments with a strong understanding of senior care needs.
Extensive knowledge of dementia and Alzheimer's care techniques.
Demonstrated leadership and management skills with supervising experience in a healthcare setting.
Proficiency in developing and managing personalised care plans.
Excellent organisational skills with the ability to multitask effectively under pressure.
Strong communication skills to liaise confidently with residents, families, healthcare professionals, and team members.
Knowledge of medication administration protocols and regulatory requirements within home care or care home settings.
