Role Overview:




Under supervision you will be expected to undertake Care Act assessments of older people and people with a physical disability and their carer's, whose needs do not require a Social Worker to organise the provision of outreach and support services as required. Also, to assist with the more complex assessments under the guidance of Social Worker / Care Manager, Senior Practitioner or colleagues in the cluster team.



An area of development in the integrated team and a priority focus is reduce the number of hospital presentations and support people to stay health, safe and well within the community.

Key Responsibilities:



To undertake Care Act assessments of individuals and their carers whose needs are not complex and do not require care management in accordance with the Department's policies, procedures and Care Act eligibility criteria. To assist Care Managers undertaking more complex assessments. To identify and take account of community networks which may assist in meetings the needs of individuals and their carers referred for a Care Act assessment. To support service users and carers in engaging in employment and occupational activities by identifying potential provision and reviewing the options available. In partnerships with the individual and their carers to prepare a written care plan, and provide advice, guidance and support. To actively promote effective communication and engagement with older people and people with a physical disability and ensure that their views are central to plans developed. To organise provision of appropriate outreach support / vocational services as required: including the commissioning of private and voluntary services under the direction of the social worker / care manager; and to keep providers advised and informed of any changes required to care arrangements. To ensure that services reflect the special needs of individuals from all protected characteristics and being aware of the additional needs of those who may be socially disadvantaged. To consult with service providers in monitoring the effectiveness of care provided, to undertake re-assessment visits when necessary, in line with the department's policies and procedures, and to re-evaluate the needs of individuals and their carers to ensure that services provided are adequate, effective and appropriate. In consultation with the social worker / care manager decide when it is appropriate to refer to a Social Worker/ Care Manager for a more detailed assessment or onward referral / service. To co-ordinate support services for service users by establishing and, maintaining effective links with relevant agencies; for example, housing partners, Education, CCG and voluntary agencies as appropriate. To ensure that electronic case files are compiled and maintained and that appropriate accurate and factual information is recorded in accordance with the department's policy and procedures. To attend liaison meetings and case conferences involving other agencies to ensure that individuals needs are properly identified and that appropriate services are provided. 13. To provide Managers with reports concerning individual cases when requested to assist in the collation of purchasing statistics ensuring that information is available and required. Under the direction of the Team Manager to undertake other tasks including cover for colleagues and delegated activities within an individual's care plan. In consultation with service users and partner agencies, to assist with the assessment of housing needs, aids and adaptations and provision.
