The Social Prescriber will support individuals within the community to improve their health and wellbeing by connecting them with a wide range of community resources and services. Working closely with GPs and other healthcare professionals within the Oldbury and Langley Network Primary Care Network (PCN), the postholder will provide a seamless referral pathway and help patients access meaningful support.
This role involves empowering clients to set personal goals, navigate local services, and engage in activities that enhance their overall wellbeing. Strong relationship-building within the community and across health and social care partners is essential to embed an effective social prescribing service.
The Social Prescriber will also contribute to developing and maintaining a directory of services, working closely with voluntary, community and social enterprise (VCSE) organisations. They will be part of a wider team of social prescribers at Health Exchange, receiving direct support from the Community Social Prescribing Lead and peer support from Community Social Prescribers.
Key Duties and Responsibilities
Build meaningful relationships with patients, investing time to understand their interests and help unlock their potential.
Provide clear information, advice, and guidance to referred clients, including appropriate signposting.
Conduct initial assessments, identify priorities, and develop tailored wellbeing plans.
Ensure all work complies with contractual monitoring, evaluation, and reporting requirements.
Record all project activity accurately and provide outcomes feedback to Health Exchange.
Attend relevant meetings to share project progress.
Work collaboratively with healthcare professionals to increase referrals and improve access to the service.
Develop a strong understanding of local support services related to social isolation, wellbeing, housing, unemployment, welfare, and benefits.
Expand and maintain a high-quality service directory for Oldbury and Langleyby visiting and quality-assuring VCSE organisations.
oBuild and maintain effective relationships with health and social care teams, advice services, stakeholders, and partner organisations.
Adapt communication methods to meet individual needs.
Contribute to the development and improvement of the Social Prescribing service, providing insight to support growth.
Manage administrative tasks and correspondence independently.
Provide cover for colleagues by delivering like-for-like support, including advising patients on social care issues, assisting with blue badge applications, and supporting form completion.
