North Islington One Limited is seeking a dedicated, empathetic, and passionate Social Prescribing Link Worker to join our team on a 13 month fixed-term contract to cover maternity leave. The role will be based at The Village Practice, one of the four member practices within our Primary Care Network.
As a collaborative network of four member practices, we are looking for a proactive individual who is committed to understanding and supporting our patients. This role is motivated by the desire to help individuals become active and independent members of their communities.
The ideal candidate will have experience in working positively with people facing complex social and emotional challenges, as well as knowledge of solution-focused coaching approaches. You will be working with people from diverse cultural and social backgrounds and must be able to work confidently and effectively within a diverse environment.
At North Islington One Limited, we expect our Social Prescribing Link Workers to identify local community resources and facilitate connections between these resources and the practice for the benefit of patients. They will also develop relationships and processes with statutory organizations, health services, and voluntary organizations such as the Citizens Advice Bureau, Money Advice Service, NHS services, and charities.
We would like our Social Prescribers to empower individuals to take control of their health and wellbeing through referrals and support the growth of sustainable community groups. The role also involves working closely with the Primary Care Network multi-disciplinary team to aim to support individuals to live well.
Responsibilities of the Role:
Provide professional and trauma-informed services to people experiencing complex social circumstances, using person-centred principles to build non-dependent, trusted relationships.
Support access to community-based services and activities that will help individuals live well, challenging service providers where necessary for the benefit of the patient.
Build effective pathways to support individuals and help them navigate complex systems.
Work autonomously to manage workload, appointments, and priorities effectively.
Maintain accurate records at all stages (pre-intervention, progress, and post-intervention).
Deliver reports in an agreed format within specified timelines.
Support the primary care team in building and maintaining knowledge of local services and activities.
Develop professional working relationships within GP practices and with local service providers, ensuring patient needs are met.
Collaborate with local Health Improvement teams and other partners to strengthen community-oriented approaches to health and wellbeing.
Enable individuals to identify personal goals and connect with relevant services to address barriers to their health and wellbeing.
Develop and deliver community groups and activities where appropriate.
Comply with all policies and procedures related to the role and maintain flexibility to meet individual needs.
Adhere to data protection regulations ensuring fair, lawful, and transparent handling of information.

Person Specification
:
Essential
Higher level of education (e.g. NVQ level 3 or A levels at minimum)
Ability to develop a non-dependent relationship, maintaining professional boundaries while being supportive and non-judgmental.
Resilience in dealing with challenging situations.
A solution-focused approach to problem-solving.
Strong understanding of the challenges faced by people living in areas of deprivation.
Excellent interpersonal and communication skills for engaging individuals and organizations.
Ability to influence and motivate people to engage with a wide range of services and activities.
Skilled in networking, information management, and building relationships.
Strong time management and prioritization skills.
Understanding of data protection principles.
Ability to work independently and as part of a team.
IT proficiency in word processing, spreadsheets, email, and internet.
Willingness and ability to travel within the local community and wider area.
Understands the importance of patient confidentiality.
Desirable
Experience with EMIS clinical systems.
At least two years' experience working in primary care, the NHS, or Social Care
Experience working with individuals facing complex social, emotional, and mental health challenges, such as chronic illness, addiction, or homelessness..
