Considerable focus is placed on Assessment and Reablement services, ensuring an equitable and integrated approach to supporting individuals from both Hospital and Community settings. By promoting independence and leveraging opportunities for early help, the team ensures that resources are prioritised for those requiring social work support. This model strengthens collaboration with internal Swansea Council teams to enable timely move-on, helping to maximise the reach and impact of reablement services for more citizens across Swansea. Through coordinated, person-centred interventions, the team supports improved outcomes and reduces reliance on long-term care.

About the Role



As a Social Services Practitioner, you will:

Work as part of a multidisciplinary team, supporting social work activity and undertaking appropriate, proportionate assessments. Provide practical and social support for individuals, families, and carers, promoting independence and wellbeing. Carry out non-complex assessments and reviews, fulfilling statutory duties and applying the principles of good practice. Support individuals to communicate their choices and maximise control over their outcomes. Liaise with professionals, agencies, and families to ensure effective care and support planning. Monitor and review support plans, ensuring they meet identified outcomes. Uphold safeguarding principles and contribute to non-criminal investigations as required. Maintain accurate records and respect confidentiality at all times.

You will be based within Hospital settings and within the Community and will work closely with the Multi-Disciplinary Team (MDT) to deliver outcome-focused, strengths-based support.

What We're Looking For



A recognised social care qualification at Level 4 or above (e.g., OU Higher Education in Social Care Practice Wales). Registration as a Social Services Practitioner (or eligibility to register). Experience in a social care or related setting (paid or voluntary), ideally within a multi-agency environment. Strong communication, IT, and report-writing skills. Commitment to equality, safeguarding, and continuous professional development. Ability to travel across sites as required (use of own car or alternative arrangements).
