We are seeking a committed and skilled Social Worker to join the Adult Services team at the London Borough of Hackney. The successful candidate will deliver a high-quality, strengths-based social work service, supporting adults, carers, and their families to achieve positive outcomes and live independently. You will play a key role in assessments, care planning, and safeguarding, working in line with the Care Act principles and statutory responsibilities. This role requires a compassionate approach, strong case management skills, and the ability to work collaboratively within a multi-disciplinary environment.
Responsibilities
Carry out strengths-based assessments of adults and carers in line with the Care Act and local policies.
Develop personalised, outcomes-focused support plans that promote independence and wellbeing.
Provide information, advice, and signposting to help people make informed decisions about their care and support.
Lead and contribute to safeguarding adults enquiries, applying Making Safeguarding Personal principles.
Work closely with health, mental health, and social care professionals to ensure joined-up support for clients.
Apply knowledge of the Mental Capacity Act, Deprivation of Liberty Safeguards, and risk management frameworks.
Maintain accurate, high-quality case records, reports, and documentation in line with professional standards.
Contribute to legal processes, including preparing court reports when required.
Promote equality, diversity, and inclusion in service delivery.
Participate in reflective practice, supervision, and continuous professional development.
Requirements
A recognised qualification in Social Work (e.g., degree, DipSW, CQSW).
Registration with Social Work England.
Knowledge of the Care Act, Mental Capacity Act, DOLS, and safeguarding frameworks.
Strong interpersonal and communication skills, with the ability to build effective relationships with service users, carers, and professionals.
Experience of assessment, support planning, and case management (ASYE applicants will be supported).
Ability to work independently and as part of a team in a diverse community setting.
Proficiency in IT systems for case recording and report writing.
