What you will do:
- Undertake a range of community care and safeguarding assessments to identify risks and needs in accordance with statutory requirements.
- Develop and implement care plans tailored to client needs, pinpointing any unmet requirements.
- Construct value for money care packages and commission services that minimise risk.
- Work closely with individuals, their families, carers, and communities to support informed decision-making.
- Manage an allocated caseload, ensuring effective monitoring and review of client needs.
- Foster relationships with clients, offering advice, promoting independence, and providing conflict resolution.
- Liaise with colleagues and external agencies for informed assessment and care planning, advocating for clients and carers.
- Maintain accurate records and report writing, with court representation as required.
- Supervise social work students, trainees, or support staff, guiding them through practical experiences.
- Continue post-qualifying professional development in line with the Care Council for Wales requirements.
