About the Advice and Wellbeing Hub

The Advice and Wellbeing Hub is the first point of contact for adults, carers, and professionals seeking advice and support. Our focus is on early intervention and prevention, working with people for up to six weeks to help them remain independent and maximise choice and control.



Our team uses the Conversations Count strengths-based model, ensuring interventions are person-centred and outcome-focused. You'll work with adults with a range of needs, including:


Older adults People with physical or learning disabilities Individuals affected by substance or alcohol misuse Carers requiring information, advice and support

About the Role:

As a Social Worker in the Advice and Wellbeing Hub, you will be the first point of contact for adults, carers, and professionals seeking support. You will undertake strengths-based Care Act assessments, conduct Mental Capacity assessments, and facilitate best interest decisions. In addition, you will manage safeguarding enquiries when required and provide information, advice, and guidance to both adults and carers.



You will manage a short-term, varied caseload focused on promoting independence, choice, and wellbeing, ensuring that every intervention is tailored to the individual's needs. Your work will be guided by the Conversations Count model, emphasising early intervention, prevention, and outcomes that enhance people's quality of life.



You will work collaboratively with colleagues, health professionals, and partner agencies, applying your professional judgement within the legal and statutory framework to deliver safe, effective, and proportionate interventions. Your role will be pivotal in supporting adults and carers to access the right services at the right time while maximising independence.



You will need to have:


A degree or equivalent in Social Work Registration with Social work England Understanding of the Care Act 2014

You will have access to our Learning and Development programme and be supported in your career and personal growth with opportunities to learn and develop new clinical skills, helping you to build your Continuing Professional Development portfolio, extending your skills and supporting you to grow as a social worker.
