Specialist Mental Health and Substance Misuse Housing Outreach Service (HOS)

The Specialist Mental Health and Substance Misuse Housing Outreach Service is a collaboration of agencies including Health, Local Authorities, and Third Sector, delivering health interventions treatment, and support directly to single homeless people placed in temporary accommodation provision across Cwm Taf Morgannwg and to individuals accommodated in the community as part of Housing First provision who are at risk of homelessness. This is a new and innovative service which commenced in 2021.
To build on the successes already achieved, and to meet the increase in demand, we are looking for a Social Worker / Experienced Social Worker who will be an integral part of the team, bringing a much-needed community care perspective. You will be an enthusiastic professional with a passion for partnership working who has a strong background and experience in working with vulnerable individuals with multi-faceted issues. You will be working directly with individuals who have been identified as having complex social care issues that are impacting on their ability to access and / or sustain a tenancy.
This post is an exciting opportunity to participate in a new area of work. You will deliver effective joint working arrangements with existing partnerships across the region; identify opportunities for development of the role in accordance with the needs of the population; raise awareness of the service and develop mechanisms for participation of people who use our services to inform service delivery and development.
You will need to be a good team player and communicator; be able to respond flexibly to changing demands and priorities; work as an individual with self-motivation and act with diplomacy in sometimes difficult and sensitive areas/circumstances to achieve challenging targets.
The hours of work are 37 per week, and the team is based within Sardis House, Pontypridd, CF37 1DU. The post will cover both the RCT and Merthyr area and there will be an opportunity for remote working and accessing bases throughout RCT and Merthyr.
The post is grant funded until March 2026. Ongoing funding will be subject to a successful bid to Welsh Government. However, the post is advertised as a permanent role within RCT.

You must possess a professional recognised Social Work qualification and be expected to be registered and maintain your registration as a 'Social Worker' with Social Care Wales on appointment.
