Job Description

We are looking to recruit an EIP Community Mental Health Social worker who is motivated and enthusiastic Mental Health Practitioner with proven abilities to work positively with service user, carers and family.



Redbridge EIP is a multi-disciplinary service, providing a range of specialist interventions within an integrated person centred approach to people aged 14-65 resident in the Borough of Redbridge experiencing or suspected of experiencing a first episode of psychosis.



EIP is an exciting community mental health specialist team that is well resourced to support the team to ensure that we offer the NICE guideline for interventions that require the collaborative working through our multi disciplinary team.

To provide an integrated recovery centred clinical service to people aged 14-65 living in the Borough of Redbridge, who are experiencing a first episode of psychosis, in accordance with the new waiting time guidelines NICE Standard 80.



To assess and act as care coordinator to clients referred to the service. Undertake comprehensive assessment of health and social needs together with the service users implement a plan of care and intervention to meet these needs, ensuring regular reviews are in place for evaluation of the care plan.



To provide carer assessments to ensure carers and family are included and supported and in care planning. To participate in carer focused education, family intervention approaches, support programmes and carers groups providing information about relevant legislation and carer's rights.



To facilitate and implement the Trust and Local Authority requirements in relation to safeguarding children and guidance legislation within the Care Act 2014. To adhere to the requirements of the Safeguarding Adults policy.



Starting with NELFT



NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.



Probationary Period



This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).



High-Cost Area Supplement



This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of 4,714 and a maximum payment of 5,941) pro rata for part time staff.

To work collaboratively and effectively as a member of the multi-disciplinary team.



To be responsible and accountable together with the multi-disciplinary team for the provision of high quality clinical care and interventions to service users, carers and family.



To ensure clinical treatment and interventions are needs led and implemented in compliance the NICE concordant Standard 80. To be p roactive, innovative and creative in delivering care, enabling the facilitation of service user's recovery, empowerment and well-being.



To ensure effective risk management by accident/incident reporting.



To be responsible for the provision of accurate recording of information required by the Trust for audit purposes and to contribute as requested by the clinical lead in the process of service audit and evaluation.



To undertake comprehensive clinical assessment of psychosis for service users referred to EIP and to formulate appropriate risk management plans.



To undertake mental state assessments, identifying any significant risk factors to support recovery and actively prevent relapse.



I n consultation with MDT team, zoning and clinical Lead formulate and implement relapse prevention plan, crisis management and positive risk taking.



In conjunction with the service user, carer and family, where consent is given, and the MDT ensure individual care plans are formulated and responsive to the needs of the service user, evidence based, compliant with NICE Concordant guidance, regularly reviewed and evaluated in accordance with CPA and the service users recovery plan.



To facilitate the delivery of care in a sensitive and professional manner respecting the customs, individuality, values, and spiritual belief of service users and their carers.



Together with service user, carer and family facilitate and coordinate effective discharge planning for service user in consultation with clinical lead and MDT.



To work closely with in-patient, HTT, CAMHs and Perinatal services, undertaking joint collaborative working as appropriate. And also with other agencies including primary care, voluntary and statutory organisations as required.



To attend and participate effectively in adult and child protection meetings, MDT case conferences, reviews and professional meetings providing written and verbal reports as appropriate.



As a social worker in the team, to be responsible and undertake duties in relation to complying with the Care act - both care act assessments and carers assessments under the care act.



As a social worker within the team to support safeguarding of both adults and children in the team



To promote and apply equal opportunities and anti-discriminatory interventions that respects the service user's customs, values and spiritual beliefs .



To proactively contribute, co-facilitate and participate in the development and delivery of EIP group and community programmes.



To undertake clinical professional supervision.



To undertake Rio Live/managerial supervision.



To report all incidents or accidents following agreed local procedures, with notification to the clinical lead and or service manager.



To deputise for the clinical lead as requested and provide cover for the work of other team members in their absence when required.



To possess a sound operational knowledge of the mental health act, its application and to comply with the requirements in the delivery of care and administration of treatment.



To provide support to the service user and carer as appropriate during formal Mental Health Act Assessments.



To be conversant with the Mental Capacity Act. To comply with the requirements of the Act in delivery of care. To provide support to the service user and carer as appropriate.



Ensure adherence to the Trust Policies, Professional Code of Conduct, Social Services and local policies and procedures including Section 117.
