We have a full-time, permanent vacancy in our Discharge Hub Team, based in Northallerton. This role focuses on supporting hospital discharges from South Tees Trust sites (James Cook, Friarage, and the Friary), as well as Darlington Memorial, Bishop Auckland General, and other surrounding hospitals for residents in North Yorkshire's northern region.





In this fast-paced and collaborative environment, you'll work alongside health professionals to ensure smooth transitions of care under the Discharge to Assess framework. You'll carry out person-centred assessments, always guided by a strengths-based approach that prioritises wellbeing and supports individuals in returning safely to their homes.





You may be involved in Mental Capacity assessments and contribute to Best Interests decisions in partnership with families, clinicians, and other key stakeholders. Creativity is encouraged as we explore safe, practical solutions rooted in a Home First philosophy.





You'll also:


Assess and support informal carers. Safeguard vulnerable adults through appropriate interventions and enquiries. Help uphold a culture of safety, transparency, and ongoing learning.
