High Needs Management Team - Social Worker Grade 8
Are you passionate about making a positive and lasting difference for young people transitioning into adulthood in Hull?
If you are inspired by a challenge and want to make a lasting difference, to support younger adults with diagnosis of Learning Disability, Autism or Mental Health we look forward to hearing from you.
We are looking to appoint at Grade 8 level to be part of a newly formed Preparation for Adulthood Team (Pfa) based within the High Needs Team in Adult Social Care.
You will work as part of an integrated multi-disciplinary team to provide high quality social work practice. You will ensure individuals have a solution focussed, strength-based plan towards achieving their goals and maintaining their independence, throughout their journey transitioning into adulthood.
You will work creatively and collaboratively with younger persons (in some cases from the age of 16) with our partners in Children's services. This is to provide a consistent approach within a social work interface in preparation for their transitioning journey into adult's services.
You will be working in a Multi-disciplinary arena with our partners in the Disability Teams, Education and special educational needs (SEND), Child and Adolescent Mental Health Services (CAMHS), alongside health professionals to empower our partners, to deliver the best possible outcomes for younger persons to create a positive introduction to our Adult Social Care culture.
We are looking for individuals who are qualified and experienced social workers who can demonstrate:
A strong value base that informs your work with young persons and adults.
A continuing commitment to quality outcomes - and to be able to talk about what good Social Work practice 'looks like'
A capacity for honest reflection and openness to learning;
Ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers
The ability to work in a multidisciplinary arena to promote autonomy and implement personalised safeguarding
Have an Understanding of the complexities involved with vulnerable younger persons.
You must be a qualified social worker and be registered with Social Work England.*
We provide a range of benefits to motivate, reward and recognise the valuable work of our employees, which include:
Work/Life Balance - The Council recognises that all employees need to balance home life with meeting work commitments through the provision of appropriate policies and support, for example, family and parental leave, flexible hours and career break schemes.

Pension - We provide a defined benefit pension scheme whereby the Council contributes towards a pension payable on retirement.
Employee discounts - There are a selection of voluntary benefits available giving a wide variety of staff discounts on goods and services from a range of suppliers via our Hull plus website.
Relocation Allowance available to minimise any financial detriment which may be incurred by newly appointed staff moving from outside the area.
Laptops and telephone for all social workers, to enable them to work smarter and more flexibly.
You must be able to provide and support the delivery of high quality social work activities, however, we have opportunities for different levels of experience as follows:
This post is for you if you have completed your ASYE and can demonstrate increased decision making autonomy in situations of increasing complexity, risk, uncertainty and challenge.
The High Needs Team provides consistent and intensive case management during the initial stages of a service being set up or where the professional view is that support can be reduced over a period of time e.g. over the first years of care. It provides a level of assurance for the management team (DMT) in terms of having oversight across packages of support which may involve a community deprivation of liberty and where there is a requirement to regularly review the restrictions which are in place to keep the person safe.
