Working in the community, you will work as an autonomous practitioner, as part of a team managing a caseload of palliative care patients and their carers and families with a variety of conditions, and often complex needs.

The post holder will work as part of the established Camden and Islington Community palliative care team across Camden and Islington boroughs. The successful candidate will provide specialist social work input for patients and families facing life limiting illness and bereavement, including the needs of the family before and after death, and for those living with serious illness, impending death loss and grief. The role involves independently assessing, planning, and managing a caseload of patients and their families, working collaboratively with other healthcare providers to ensure holistic and coordinated care. They will act as a specialist resource for consultation and teaching within the trust and outside.
As a service we provide palliative care to the patients of Camden & Islington. The community team is based at St Pancras Hospital and has evolved from having one consultant nurse and a medical consultant in the last 25 years to a multi professional team of nearly 30 staff, including, CNSs, Consultant Nurse, Drs, Social Workers, OTs, Physiotherapists and amazing administrative colleagues too.
