Monday to Friday, 8:30 AM 4:30 PM or 9:30 AM 5:30 PM (On-call duties as required)

Summary (Main duties and responsibilities)

As a

Specialist Clinical Pharmacist

, the post holder will play a pivotal role within the pharmacy team, contributing to the delivery of a

safe, effective, and patient-centred pharmacy service

across the Trust.

Key Responsibilities include:

a) Clinical Ward-Based Services:

Delivering a high-quality, patient-focused clinical pharmacy service on wards, ensuring the safe and effective use of medicines.

b) Supporting Timely and Safe Discharges:

Facilitating early discharge and reducing hospital length of stay by:

+ Optimising patients' drug therapy.
+ Providing tailored education on medicine use, including support with self-medication and assessing the need for adherence aids.
+ Offering comprehensive counselling to patients about their medications prior to leave and discharge.
+ Ensuring continuity of care through robust discharge planning and communication with community healthcare providers.

c) General Pharmacy Duties:

Participating in wider clinical pharmacy and departmental responsibilities as required, supporting the overall objectives of the Trust's pharmacy service.
