Job Description

An exciting opportunity has arisen for the role of a band 6 Specialist Practitioner to the Crisis Intervention Team for Older People (CITOP) due to the expansion of the team's current role. The CITOP team supports adults predominately aged over 65 in the Hull and East Riding area with both functional and organic illness.



Moving forward the CITOP team will be extending their operating hours to provide 24-hour Crisis intervention and Home treatment for Older adults across Hull and East Riding of Yorkshire. CITOP will be provide four Community Crisis Beds for older adults as an alternative to inpatient care and The Acute Community Service will be extended to provide a seven-day service.



Due to the development of the CITOP service we are looking for professionals who are keen to lead on innovation, developing robust care pathways and enhance the patient and carer experience.



We are seeking individuals who are motivated, enthusiastic and have experience of working with Older Adults who have complex needs and enduring mental illness. This role requires good organisational skills, being able to demonstrate an ability to prioritise and delegate work effectively, and an interest or specialism in urgent and emergency mental health care, providing expert clinical advice and support and provide leadership and guidance on shift.



The applicant will be expected to have specialist nursing knowledge of working with older people with organic and functional mental health issues.



You will have the opportunity to develop leadership skills, providing supervision to healthcare staff. You will lead and participate in the development of staff through appraisal and clinical supervision.



You will provide nursing interventions across Hull and East Riding, to support the wider community mental health services as required by patient need and demand. You will provide Home Treatment as an alternative to hospital admission or to facilitate a timely discharge from inpatient settings. You will be required to triage and assess patients who require a urgent or crisis response. These assessments will be completed in a range of community setting. You will be required to collect/collate information and attend formulations for identified patients. You will also be required to complete 'gate keeping assessments' and liaise with the bed management team around admissions. You will be required to work in collaboration with other teams and agencies to ensure the best care for the team's patients.



The applicant will be expected to have knowledge in mental health conditions, including Dementia and experience in this area would be an advantage, additional training and support will be given.



We are a 24/7 service therefore patterns of duties include days, evenings, weekend's, nights and bank holidays.
