An exciting opportunity has arisen for the role of a band 6 Specialist Practitioner to join our Mental Health Crisis Intervention Team. We are an urgent care service for adults aged 18-64 (and their families and carers) living in Hull and East Riding who are experiencing an acute mental health crisis.
We work closely with our partner agencies including Yorkshire Ambulance Service, Humberside Police, Secondary, Primary and voluntary care providers to deliver Crisis and Home Based Treatment support and offer a gatekeeping service for Inpatient care.
We are seeking professionals with a Mental Health or Learning disability nursing registration or social worker with experience of working within the specialism of Mental Health and we are looking for professionals who are keen to lead on innovation, developing robust care pathways and enhance the patient and carer experience.
We are seeking individuals who are motivated, enthusiastic and have experience of working with people who have complex needs and enduring mental illness. This role requires good organisational skills, being able to demonstrate an ability to prioritise and delegate work effectively, and an interest or specialism in urgent and emergency mental health care, providing expert clinical advice and support and provide leadership and guidance on shift.
The main features of this role include assessing the care needs and developing and implementing plans of care for individuals who are experiencing acute mental health deterioration or who are in a state of mental health crisis.
Collaboration with patients and carers and partner agencies is a key element to delivering high quality crisis and acute interventions.
We provide crisis assessment and Home Based Treatment Interventions, gatekeeping for inpatient admissions, coordinate and manage the section 136 suite/Mental Health Based Place of Safety pathway at Miranda House and the 111 option 2 Urgent phone line. We are due to commence partnership working with the Mental Health response Vehicle with Yorkshire ambulance paramedics which is a vehicle to provide an effective response to people in mental health distress who have called 111 or 999 and require a face to face response. Our work is diverse, fast paced and clinically rich.
We are a 24/7 service therefore patterns of duties include days, evenings, weekend's, nights and bank holidays.
