Job Purpose



To provide independent living and enable the people we support to live in their own home as independently as possible. Our aim is to support individuals to lead their best lives and a life full of meaning and purpose.

As an enhanced support lead, you will be allocated to support individuals who may have a mental health diagnosis, dual diagnosis, Autism and people who may portray behaviours of concern. You will have experienced understanding of the rationale, formulation and function of individuals behaviours of concern and provide an evidenced base and well-informed skilful response.

You will play a part in coaching and teaching individuals to lead their best lives and provide leadership and mentoring to teams and colleagues when on duty.

You may be required to work with individuals who are resettling from hospital settings into the community and support in providing bespoke environments, care and support tailored to their best interests, needs and wishes.

You will be required to work shift patterns, weekends and public holidays, participate in management on call systems and attend meetings.

You will ensure that you fully understand and fulfil your responsibilities in respect Health & Safety, Care planning, required training and Diversity & Inclusion at all times.

Key Accountabilities



Support managers in rota planning, where appropriate plan shifts and allocate tasks to colleagues on duty. Ensure the health and safety of people we support, colleagues and other people is a priority. Be an effective team player with a willingness to learn and lead the values of our organisation. Ensure Daily Diaries and all communication is completed to the expected standard at the end of the shift.
5. Be committed to ensuring and leading rights-based, strengths-based support and ensure a PBS agenda is implemented throughout the service, providing and leading in person centred care to the individuals you will be supporting and mentoring colleagues to actively do this.

Be able to learn about an individual's specific needs and provide/lead in providing support in the most appropriate way.
7. Provide sensitive and responsive support and be mindful of a trauma informed approach through providing resilience, consistency and continuity.

Check appointments for the day for all individuals in the service and make sure they are met. Ensure activity plans for all persons in the service are meaningful and evidence choice, are in place and are adhered to by all colleagues. Communicate all important issues, incidents, concerns and safeguarding to management professionally and promptly, and are recorded. Inform the line manager of any incidents on the shift before the end of the shift and ensure the correct documentation has been completed to a good standard and submitted. Take responsibility for and make sure appropriate decisions are made, reported and documented, regarding any issues on the shift. At the end of the shift check to make sure all tasks allocated have been completed Keep an up-to-date record of medication, including medication received, administered and stock kept. There must be a clear audit-trail attached to every medication and when it is disposed of there must be an official receipt from a Pharmacy verifying this fact. Support in ensuring an up-to-date risk assessment is in place for every task and activity undertaken by all people we support at the service and colleagues. Ensure there is an inventory of all persons we support at the service, detailing their possessions and ensure they are maintained in good order. Record and count all person's we support at the service, personal or other monies in-line with Policies and Procedures and safe-guard their finances from fraud, theft abuse and deception by making sure Policies and Procedures are implemented and strictly adhered to. Protect the people we support and the organisation by accounting properly for money and produce accounting records as and when requested but in any event at the end of each day. Write shift reports containing factual, accurate and detailed reports that provide information about all aspects of each person's day in a valuing and positive manner. Ensure that all pertinent information is recorded such as marks and bruises, and how
they are treated. Daily Diaries must evidence that a high standard of care and support is provided and any failures must be properly recorded and reported. There must be an audit-trail running through Daily Diaries, Communication Books, E-mail Communications, Incident reports.

To support with management on call systems beyond contracted hours. Provide evidence of and maintain professional qualifications and experience of working with individuals with complex mental health conditions and other multiple diagnosis such as Autism.

Location



This role is not allocated to one site; Jigsaw Creative Care will allocate working locations based upon the needs of individuals supported and skill-set of colleagues.

You may be required to work at or from any additional location as may be determined by Jigsaw Creative Care. Jigsaw Creative Care also reserves the right to change your working location on a permanent basis to a location determined by it at the time that the change is made.

You are required to travel between Jigsaw Creative Care services as may be required for the performance of your duties.

Flexibility



Jigsaw Creative Care reserves the right to amend your job description and/or your duties. You also agree that you will work with Jigsaw Creative Care to deliver Jigsaw Creative Care's services, including without limitation, by complying with lawful and reasonable instructions from Jigsaw Creative Care by adapting to new ways of working and attending training courses determined by Jigsaw Creative Care.

Continuing Professional Development



To attend and contribute to colleague meetings and Forums, supervision sessions, training courses, seminars and workshops, to ensure the development and enhancement of current working practices.

To participate in all personal review meetings, and take responsibility for personal and professional development. Responsibility for developing the role and the services provided.

GDPR



You should be aware of the Data Protection Act and follow the local Codes of Practice to ensure appropriate action is taken to safeguard confidential information.

Health & Safety



You are required to take responsibility for Health and Safety Risk Assessment and Workplace Inspections, and to take reasonable care for your own health and safety and that of other persons who may be affected by his/her acts of omissions.

You are also required to lead and co-operate with all colleagues to ensure that all statutory regulations, policies, Codes of Practice and departmental safety procedures are adhered to, and to attend relevant training programmes.

Infection Control



You must ensure that you and colleagues follow policy and procedures aimed at reducing the risk of passing on the organisms that can cause infections, and remain up to date in training relating to Infection Prevention and Control.

All colleagues, collectively and individually, have a duty of care in following best practice in adherence to guidelines which is a fundamental requirement in underpinning the management of Infection Control.

You must return to Jigsaw Creative Care upon request and in any event upon the termination of your employment, all documents and tangible items which belong to Jigsaw Creative Care or which contain or refer to any confidential information and which are in your possession or under your control.

A breach of confidence could result in dismissal.

You should be aware that regardless of any disciplinary action taken, a breach of confidence could also result in civil action for damages.

Safeguarding



Safeguarding of individuals who come into contact with our services, whether a child or young person, person with Learning Disabilities or an older or vulnerable adult, is the responsibility of all employees of Jigsaw Creative Care in whatever capacity they are employed. All colleagues must read and adhere to the Safeguarding policies and procedures in place.
