Job Description

The post holder will assist with the delivery of pre-assessment service at Moorfields City Road site.
The post holder will facilitate the provision of excellent nursing care and work collaboratively with the multidisciplinary team. They will assist in ensuring the effective running of the service whilst ensuring the safety and wellbeing of patients and always working in a manner that promotes dignity and human rights through the adoption of person centred care principles.
Main duties of the job
Responsible for assessment of care needs of patients, planning programmes of care, implementation and evaluation of these programmes with direct supervision. They will apply a holistic approach to patient care, ensuring that high quality, current evidence-based nursing assessment, care planning, implementation, interventions and evaluations for patients is provided from admissionthrough to discharge.
To act as advocate for patients within the area, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
To ensure that patients' receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.
Recognise the significance of observations made, and use them to develop and initiate nursing assessments and devise a plan of care.

Job responsibilities
Responsible for carrying out clinical ophthalmic practice within designated clinical areas, ensuring that high quality, current evidence-based nursing assessment, care planning, implementation, interventions and evaluations for patients is provided from admission through to discharge
Will be responsible for the application of knowledge across the range of ophthalmic work procedures and practices, underpinned by theoretical knowledge and practical experience.

Undertake prescribed physical observations specific to ophthalmology
Responsible for the correct administration of prescribed medication and monitor effects.
Complete baseline observations including blood pressure, pulse, SpO2, capillary blood glucose, HbA1c testing, etc. Job description
