We have an exciting opportunity for a Streaming Practitioner to join our team based at Beckenham Beacon UTC on a part time basis.
Job Summary:
The post holder must be a Registered Nurse or Paramedic and is therefore bound by the NMC Code of Professional Conduct for Nurses, Midwives and Health Visitors or Health Care Professions Council. PHL group are a leading healthcare service provider in the UK and Ireland, working in partnership with the NHS and other providers to deliver healthcare services through its Urgent Care, Planned Care and Insourcing Divisions.
Key Responsibilities:
To practice as a Streaming Practitioner within the boundaries of their scope of practice.
To assess patients, collate health information and make safe and accurate clinical decisions regarding a patient journey in Urgent and Emergency care.
To be responsible for the assessment of care needs without supervision
To support and teach other nursing and non-nursing staff.
To work flexibly and rotate on to night duty.
Clinical Duties
Ensure that patients receive high quality clinical care
Request diagnostic tests including x-rays/urine. IMER training available.
Ensure that effective discharge planning and health education/ promotion.
Signposting to other health services according to clinical need.
Adhere to organisational targets for streaming and consultation
Adhere to national targets e.g. 4 hour wait and KPIs of the UTC.
Ensure accurate and systematic documentation of all clinical information given.
Establish and maintain effective communication with nursing, medical and other healthcare care professionals to provide optimum patient care.
Communicate effectively with patients, relatives, and colleagues.
Reflect on audits, taking action to address improvements as required.
Administer through PGDs medicines as appropriate according to best practice, current guidelines and legislation
Have a good knowledge of safeguarding policies in relation to both children and adults to ensure identification of vulnerable patients
