Support Worker - Day Opportunities & Home Care



Supporting Adults with Learning Difficulties and Autism



We have an exciting Job opportunity to work across both our Day opportunities and Home care service, based in Stone.



Mondays & Fridays 13 hrs per week (8:45am - 3:15pm) Day opportunities



plus



One evening/overnight shift per week 3pm - 11pm Sleep in 7am - 9am Home care



Salary 12.50 per hour

Futures2gether is a bespoke support service providing a variety of care and support to adults with learning difficulties and Autism. We work with individuals in a group training environment providing activities and training to enable individuals to learn and develop skills to assist them in all aspects of life. We also provide support to individuals living in their own homes, assisting with independent living tasks and supporting social and emotional well being.

The organisation's ethos is one of putting the client at the centre of everything we do, providing creative, tailored, person centred support to enable them to achieve their goals.

This is a permanent position, based on a successful probation period.

Support Worker Job role would include:



Organising and Planning Activities within the Day Opportunities programme Supporting the Managers and Senior Support workers to deliver a quality, person centred service. Responsible for the effective running and delivery of the day service programme alongside the senior support workers Provide person centred support and activities whilst using a secure digital app to log information and keep up to date with Client needs. Promote the independence of people we support and facilitate their involvement in decision making by championing the 5 principles of the Mental Capacity Act 2005 Delivering and Supporting a full range of activities including cookery, events planning, cafe skills, performing arts, retail skills, community events, leisure activities Working as part of a dedicated team of staff to provide a quality service Promoting the independence of people we support and facilitating their involvement in decision making Developing and nurturing positive, supportive relationships with clients and their families Carrying out health and safety responsibilities throughout the organisation including risk assessments Seek training opportunities and undertake mandatory training both online and in person. Attend training days/afternoons. Problem solve in a flexible, creative and proactive way Use own car to transport Clients where necessary (mileage will be paid)

Home Care support role includes:



Promoting choice and independence of people in their own home Work in a person centred way and understand the application of person centred practices Supporting individuals to complete a variety of home care tasks including budgeting, cooking, shopping and administering medication Promote individuals well being and access and respond to changes in well being Transporting individuals to activities, groups etc. (Drivers licence and own transport required) Supporting Clients to access activities within the community i.e the gym, swimming, meals and trips out Carrying out risk assessments and promoting positive risk taking to build skills and confidence. Collaborating with Clients, families, carers, and involved professionals to provide a coordinated and consistent service that maximises outcomes Be responsive to the needs of service users in line with Care Plans and respond flexibly to changing needs Encouraging and supporting Clients to express their needs, views and concerns and enable them to make their own choices and decisions Developing and promoting good relationships with the clients we work with and their families Promote personalised, quality care and support across the service at all times Promote equality, diversity and inclusion within the work setting Follow protocols, policies and procedures Overnight Sleep ins

Previous experience in Health and Social care is welcomed but not essential.
