This is an exciting opportunity to join a care team providing one-to-one support for our 17-year-old client who is a sociable youngster with a love for music and horse-riding.
Our client sustained a brain injury at 10 months and as a result has complex additional needs. He requires support with daily tasks such as personal care and supervision while eating. Although he is mobile, he can be unsteady on his feet and needs supervision.
As our client enjoys lots of different activities, he has a full schedule and is at the heart of a busy home. We are looking for someone who will promote independence and provide support and encouragement to enable him to reach his full potential.
As part of this role, you will organise fun, educational, and age-appropriate activities for him to enjoy with his family, friends, and care team.
Supported by a team of professionals, this is a great opportunity to develop your skills, while contributing to his therapy and educational programme which is incorporated into his day-to-day activities.
This role will suit a person with previous experience within a similar position. The ability to support children with special education needs is essential and as you will be working in a busy school and family home, it is important that you are sensitive to the needs and boundaries of the family.
Key Tasks and Responsibilities:
Maintaining all aspects of personal care as the client is totally dependent on support with his activities of daily living.
Specific
:
Developing an understanding of the specific client needs required.
Working to support learning skills by helping to complete education tasks set within school.
Promote independence and offer encouragement with tasks.
Liaise with the class teacher and other professionals about individual education plans (IEP) and Educational Health Care Plans (EHCP).
Supporting therapeutic programmes working with the therapy team.
Checking and cleaning of equipment.
Maintaining safety in all environments.
Attending care team review meetings.
Car driver.
Take and attend activities/social groups
Maintaining general health.
Some cleaning, laundry and other domestic tasks as required by the client may be involved.
General:
Encourage, monitor, and maintain health, safety, and security at home and in school.
Promote effective communication and relationships.
Contribute to the protection of individuals from abuse.
Establish and maintain a trusting, therapeutic, working relationship.
Be responsible for the client's personal welfare.
Travel to hospitals or elsewhere for therapeutic care.
Liaise with other care agencies as required
Complete daily records as advised including time sheets, daily record sheets, medication sheets, petty cash sheets and critical incidents
Attend review meetings with the employer/parents and other persons as nominated by the employer.
Attend training sessions as arranged.
