About the role
As a PRT Support Worker, you'll play a vital part in supporting vulnerable adults in the community, helping to prevent homelessness and increase independence. You'll manage a caseload of clients, delivering high-quality, person-centred support to help them achieve positive outcomes.
Your work will include:
Delivering tailored support to clients with a wide range of support needs which may include mental health, learning difficulties, substance use, and complex needs.
Creating, implementing, and updating Support Plans to achieve timely and effective outcomes.
Providing practical housing-related assistance, such as helping clients maximise income, apply for welfare benefits, prepare for or find work, manage household tasks, reduce isolation, and access other services.
Working creatively and flexibly to engage clients, identify unmet needs, and signpost or escalate as appropriate.
Communicating sensitively and effectively, supporting clients to manage emotions while maintaining a focus on practical outcomes.
Promoting independence, skills, and confidence in clients.
Maintaining accurate electronic records and contributing to case reviews and meetings.
Contributing to service improvement, performance targets, and the induction and training of colleagues.
You'll be part of a supportive team, managed by the Private Renting Team Leader, and will have the opportunity to make a real difference to people's lives across Bristol.
