LDA Hospital at Home is an innovative new service being developed under the Thames Valley Provider Collaborative. The Hospital at Home team will focus on young people with moderate to severe learning disabilities AND/OR a severe degree of functional impairment associated with an autism diagnosis who are at risk of home or placement breakdown and who could meet the threshold for inpatient care.



In some cases, the team will help prevent unnecessary admissions and in others, young people will be stepped down from tier 4 care through the LDA Hospital at Home team, shortening their inpatient admission. LDA Hospital at Home maximizes the use of new technology, provides care close to home, reduces admission, length of stay, waiting times and provides earlier discharge from inpatient care.



By managing the care pathway, we will be able to ensure that children and young people are cared for in an inpatient setting or via an 'out of hospital' service as close to home as possible. This will:



Reduce the burden on families to travel long distances



Enhance continued engagement with community clinical teams



Encourage early therapeutic home leave



Minimise stay length



Avoid admission



As the family therapist in the team you will be able to bring your perspective to complex cases and situations to the LDA H@H team and the local teams, agencies and networks around the young people on the teams caseload. At times these cases may cause high levels of emotions and impact on individuals, you will also be integral to holding and supporting the team using a systemic framework on these cases.



You will be based in our new offices in the Meadow unit on the Warneford hospital site, there will be LDA Hospital at Home is a specialist service working with patients up to 18 years old with moderate to severe learning disability and autism. The Thames Valley Provider Collaborative is a network of service providers covering Oxfordshire, Buckinghamshire, Berkshire, Gloucestershire, Swindon, Wiltshire and Bath and North East Somerset (BaNES), working collaboratively to provide high quality care for the most unwell young people as close to home as possible.



We are currently recruiting our multi-disciplinary team (MDT) who will work with young people diagnosed with moderate to severe learning disabilities and autism, who are at risk of admission due to mental health difficulties.



You will be based in Oxford in our new offices located above the meadow unit on the Warneford site.

This is an exciting time to be part of the development of a new Tier 4 service for CAMHS. The LDA H@H team will have close links and working relationships across the Thames valley collaborative area to support existing provision across the patch. The main functions of the team are to prevent admission and facilitate earlier discharge.



We provide, accredited supervision by senior therapists to help you consolidate your therapy skills in a focused and supported way, monthly opportunities to meet psychological therapy colleagues from across the county for reflective practice groups, clinical updates, and professional development.



To provide a quality family therapy service to the clients in the Hospital at Home service.



To provide consultation, supervision, and support the work of colleagues providing care and treatment, according to expertise.



To work autonomously within professional guidelines and the overall framework of the team's policies and procedures.



To utilize skills for audit, policy, service development and research



We would really like to hear from you, if you have skills in delivering therapy to this client group and can demonstrate:



Evidence of significant knowledge in your specialist therapy area



A good working knowledge of effective treatments,



Sound clinical risk assessment and management,



Experience of supervision with therapists and other clinical staff



Experience of leadership and management of therapy staff.
