The ideal candidate must have a Level 3 Health and Social Care.

You will be supporting service users with learning disabilities, mental health problems and challenging behaviour.

Purpose of the role of the Team Leader



To work as part of the management team, using person centred approaches to provide a high quality service for people with learning disabilities, which enables users to live with a maximum of choice and independence, participating in a wide range of community based activities, designed to meet their individual need. To provide professional and operational management to the team/s to assist service users to lead fulfilling lives and develop and use their individual skills as far as they are able to in all aspects of daily living in line with the Personalisation Agenda and other relevant regulations i.e. Mental Capacity Act, DOLS etc. To make professional decisions in the interest of the service users, both on a day to day basis and in emergencies, e.g. dealing with urgent medical issues, other emergencies or unanticipated difficulties etc. To maximise community inclusion and participation through enabling the involvement of service users in ordinary mainstream activities and services, ensuring that risk is managed appropriately The post holder will be required to work shifts including weekends, bank holidays, sleeping in and occasional waking nights on a rota shift system as directed.

Main duties of the Team Leader



To organise and manage the day to day running of the service, including leading shifts, allocation of duties and ensuring adequate hand over of shifts. To assist the Deputy and Service Manager in the management and control of all aspects of the service. To assume responsibility for the service in the absence of the Service and/or Deputy Service Manager ensuring an effective delivery of service is maintained and deputise for Management at meetings as required. To supervise, guide, appraise and develop staff and monitor their performance with regard to attendance levels and disciplinary procedures. To compile staff duty rotas in order to ensure adequate cover within the resources available with guidance from the Deputy/Service Manager. To provide practical and personal support to enable service users to have more control over their lives and to meet their assessed need by maintaining and promoting their health, well being and social development. To ensure service users are supported in a varied range of activities which promote the development of people as individuals, increase independence and maximise community inclusion as identified both through day to day choice and their agreed care plan. Encourage service users to make informed choices and have new experiences within an agreed plan of risk taking Assist in the provision of the highest possible standards with regard to quality of life and a homely environment including support for those users who represent a challenge to the service. To ensure all service users have allocated key workers working in partnership with service users and their families, and that staff are supported with their key worker responsibilities and to undertake key working responsibilities as required. To support service users and staff in planning and organising social, recreational and leisure activities, planning journeys, holidays, attending appointments and accompanying service users on such activities. To assist service users with their support needs as appropriate, ensuring awareness of each individual's culture, dignity and showing respect at all times. This may involve prompting to do key activities of daily living such as washing, dressing, feeding, or administering medication as required. Encourage and assist service users to look after their own finances. To support service users in developing and actioning outcomes from their person centred plan and care plan ensuring progress on outcomes are recorded. Participate and assist service users in cooking, shopping, cleaning, gardening, laundry, ironing and household tasks. Participate in achieving the highest domestic standards with respect to cleaning, hygiene and provision of meals for well balanced diets that meet the cultural and dietary needs of the residents. Assist service users to have access to and receive health care. ? Help promote the positive participation of relatives, friends and volunteers in the lives of the individual service users. Assist in identifying and minimising any risk through the process of Risk Assessment and Risk Management whilst promoting service users independence. Work in the framework of a staff rota, based around the residents' needs and the requirements of the organisation. Undertake sleep ins as required. To assist the Deputy and Service Manager in the administration and operational needs of the service, its finances, maintenance of the building, equipment, security and health and safety. To actively participate in staff development processes and work with the Deputy and Service Manager to organise activities to ensure the learning and development needs are met within existing resources. To participate in all meetings, contribute to note taking and chairing as required. To write and present all reports as required for service users and undertake other administrative duties where necessary. Ensure all necessary records are maintained e.g. daily records, residents' files, health and safety documentation, within the Council's policy and legal requirement. To assist the Deputy and Service Manager in managing change and implement agreed new ways of working to develop services. To attend and participate in supervision and appraisal meetings with line manager The post holder will actively pursue continued professional development and will seek to continue learning and development in the changing national and local policy context. The post holder must at all times carry out his/her duties with regard to CQC National Minimum Standards for Care Homes, Esto Care's Customer Care and Equal Opportunities Polices and in accordance with Esto Care's Mission Statement and Values. To undertake any other relevant duties of a similar nature and level as required.

Requirements of the Team Leader



This post is service based but will involve some out of office activity and travel across to other sites. Ideally in date DBS, but this is not a must. Previous training ideally in the following: Health and Safety, Infection control, Food hygiene, First Aid, Medication administration, Fire, Moving and Handling, safeguarding of vulnerable adults (SOVA). A positive attitude, friendly personal and a caring nature. Level 3 Certificate. Drivers Licence.
Must be able be flexible with their work hours as stated on their Contract.
