Job Role

Team Leader

Adhering to all Company Policies and Procedures within the defined timescales. Flexibility to work additional hours on occasions to meet the needs of the service.
Carry out any other duties as assigned by the Management Team or Deputy Manager to ensure the efficient running and continued development of the Home.

Key Skills:



Leadership & Teamwork

: Experience in managing care teams and promoting a collaborative environment. Keep up to date with all Service Users support plans, risk assessments and reviews and report any changes immediately to the Registered Manager.

Care Planning

: Expertise in creating and implementing care plans tailored to individual residents.

Medication Management

: Proficient in administering medication and monitoring its effects. Administer Service Users prescribed medications in line with Victoria House's Medication Policy and Procedures. Ensuring that all medication is signed into the building and securely stored in service users individual medication cabinets. Monitor repeat prescriptions with stock levels and only re-order when required.

Risk Management

: Ability to assess and mitigate risks in a care setting.

Dementia and Alzheimer's Care

: Knowledge and hands-on experience in specialized elderly care.

Safeguarding & Compliance

: Strong understanding of safeguarding policies and regulations (e.g., CQC standards).

Support of emotional and spiritual needs

of all service users and at all times aim to optimise their quality of life, whilst empowering them to retain as much control over their lives as possible. Reporting any ill-health amongst service users and make requests for GP / Professional visits where necessary.

Responsibilities

: key responsibilities such as:

Supervising junior care staff and ensuring high-quality care. Leading shifts, managing daily routines, and responding to emergencies. Creating and updating individual care plans. Conducting assessments of residents' physical and mental health needs. Administering medications and ensuring proper documentation. Ensuring compliance with health and safety standards. Liaising with healthcare professionals, families, and residents to ensure holistic care. Training and mentoring new staff members.

Achievements

: Highlight any specific contributions such as reducing incidents, improving care quality scores, or successfully leading inspections.

Qualifications:



Health and Social Care Diplomas

: NVQ 3, or higher in Health and Social Care or equivalent.

Care Certification

: Completion of mandatory care training and courses (e.g., Care Certificate).

Medication Management Certification

: Evidence of specific training in administering medication.

First Aid & CPR Certification

: Proof of up-to-date first aid and CPR training.

Specialized Training

: Any additional training relevant to elderly care, such as dementia care, palliative care, or mental health support.

Communication:



Communication

: Strong verbal and written communication, including liaising with families, medical professionals, and other staff. Accurately record detailed information on Service Users daily record log during and at end of shift, ensuring that all information of a confidential nature gained in the course of work is not divulged to third parties. Practicing maximum integrity in all dealings with service users personal and financial affairs when required. Notifying the Deputy Manager or Home Manager as soon as possible of your inability to report for duty, and also on your return to work from all periods of absence. Informing the Home Manager of any incident / accident, and the status of any service users whose care / health is causing concern. Supervising visits and liaising with GPs in order to establish a good relationship between GPs / Health Care Professionals and the Home Ensuring a clear and concise Handover Report is given to all Staff Members. Arranging / participating in Staff and service user meetings as and when required.

Health & Safety:



Ensuring the security of the Home is maintained at all times. Understanding and ensuring the implementation of the Victoria House's Health & Safety, Infection Control and Hygiene Policies, and Emergency & Fire Procedures. In the absence of the Home Manager, carrying out duties for the Home in line with Care Quality Commission guidelines, the Health and Safety at Work Act and Fire Regulations. Promoting safe working practice within the Home. Reporting to the Home Manager or the Handy-person, of any faulty alarms, appliances, damaged furniture, equipment or any potential hazards. Ensure Service Users rooms are sustained to a high standard of cleanliness and remain hazard free in line with all Health & Safety policies and procedures. Always wear appropriate uniform and correct PPE when required.

General:



Ensuring all Staff Members contribute to the best of their ability to the efficient running of the Home, and to the creation of an atmosphere conductive to the best interests of Service Users. Interacting with Service Users to ensure that care and support needs are being met and resolving any problems where appropriate. Preparation of light snacks and beverages for Service Users, ensuring meals are of sufficient quantity and good quality, and that service users dietary needs are met. Practicing maximum integrity in all dealings with service users personal and financial affairs when required. Ensure all mandatory and service specific training is up to date. To carry out weekly allocated audits and record and report any concerns / issues to the Deputy/Registered Manager. Liaise with the community Pharmacist and GPs to promote effective medicines management so as to optimise therapeutic outcomes from medicines for all service users. A high standard of Personal hygiene and appearance is essential at all times. Support individuals and their families with End of Life care.

Experience:

Supervisory

: 2 year as Sr. Care with medication management (Preferred)

Qualification

: NVQ 3, or higher in Health and Social Care or equivalent.

IT Skills

: Familiarity with care management systems or electronic care plan software.
