Job Description

The post holder will carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance.



The Manager will develop and coordinate a multi-professional approach

providing evidence-based care to all those who use the service.

This collaboration will harness an integrative agency approach so that physical, psychological and social care agencies are working together in providing holistic care to individuals who require its support.

The post holder will be responsible for the continued development of the CLDT and will ensure service delivery is of a high professional standard, in line with Health Board policies and procedures.



To work within the policies, procedures and guidelines set out by the Health Board and professional bodies. Operate within the guidelines set by the NSF, Clinical Governance, Risk Management and NICE guidelines.



Establish and maintain effective care through the holistic and comprehensive assessment of service user's needs, planning, implementation, and review in accordance with PTHB and relevant Learning Disability policy and procedures.
