Job Description

FULL TIME Team Leader role



Supreme care Redditch is looking to recruit a Team Leader in

Bromsgrove, Redditch & Alvechurch

area to work in our home care service for various shift patterns.

The primary purpose of the Team Leader role is to ensure the consistent provision of

high-quality, person-centred care

to all service users. This includes actively championing the rights, privacy, and dignity of every service user, and ensuring their continuous involvement in both the planning and delivery of their care and support. The Team Leader acts as a key link between the Care Manager and the Care Assistant team, driving compliance and best practice on the ground.

Key Responsibilities:



Staff Support and Development



To

support Care Assistants

through mentorship, guidance, and direct assistance, fostering a positive and professional team environment. To conduct

shadowing sessions

with new team members, ensuring a comprehensive induction and orientation to care practices. To provide training and support on the use of the

Care Management App

, ensuring all staff are proficient in digital record-keeping.
Create and uphold an open, positive and inclusive management culture.

Work to establish effective employer-employee relationships.

Promote the effective resolution of team conflicts.

Quality and Compliance



To conduct

spot checks

on staff during client calls to ensure strict adherence to all established procedures, policies, and quality standards. To complete

medication audits

for clients on a routine basis to ensure accuracy and compliance with prescribing instructions. To complete

medication observations

on staff to assess competency and adherence to safe administration protocols. To ensure all clients'

Care Plans are up to date

, accurately reflecting current needs, risks, and personal preferences, reporting back any changes to the Care Manager without delay.
Team Leaders must be maintaining the confidentiality of the service users and other staff members all the time as it is zero tolerance as per our company policy.

Comply with Infection control policies and procedures of the company.

Client Welfare and Safety



To proactively

monitor the client's condition

and well-being, reporting any changes, deterioration, or immediate concerns to the Care Manager without delay. To contribute to the

maintenance of a safe and healthy environment

within clients' homes, identifying and mitigating potential hazards. To report

all accidents or incidents

involving staff or service users to the Care Manager immediately and accurately, following established incident reporting procedures.
Any service users returned from hospital, active list of medication on the discharge summary to be compared with the MAR sheet and update MAR sheet if required.

Rota and Collaboration



To comply fully with the

On-Call Rota Management

system as devised by management, including participation in at least

one on-call duty per week

and commitment to

every other weekend, including Bank Holidays

. To actively

participate in all staff meetings

, contributing to discussions on service improvement, operational issues, and team development.
Team leaders are expected to be available on their On-call day to cover any gaps in the planned Rota arising from unforeseen circumstances (i.e. Sickness, absenteeism, car-breakdown etc.).

In case of any changes to the planned Rota (call timings and staff replacement), the same need to be communicated with the impacted staff and service users effectively and also need to be documented in the handover sheet.

Generate a detailed handover report at the end of On-Call shift and share/discuss with the manager.

On-call phone must be answered without fail. In case of any missed call, the call back timeline is 30min max.

Requirements

You need to have a full UK driving license as well as having your own car and live in Redditch/Bromsgrove area.

Be available to work weekends.
