Job Description

Job Purpose:

To support the Nautilus Care Manager in delivering high quality, personalised care ensuring that all current standards are met.

Ensure the effective day to day operation of the Nautilus Care Service. To be responsible for the coordination of the home care service, ensuring the effective promotion of the health and wellbeing of all residents. To be responsible for the assessment and review of all care packages and supervision of staff.

Key Accountabilities:



Provide high quality care including administering medication to individuals assessed as having support needs.

To support the NCM in ensuring that all Care Plans are up to date on the Person Centred Software System (PCS), tailored to the individuals we care for are regularly monitored, audit and reviewed.

Support the NCM with Quality Assurance of the Nautilus Care Service to ensure they meet the regulatory and legislative framework.

To liaise with the NCM on any issues regarding the legislative requirements concerning the services provided and the staff providing them.

Support the performance and development of homecare staff. Provide direction, personal development and motivation necessary to secure the optimum commitment and competency of the team to ensure Nautilus Care operates at maximum effectiveness.

Order and check in monthly medications and complete regular MAR sheet checks and staff competency checks.

Ensure that families and relatives are regularly updated and consulted re the care we are providing.

Ensure the effective rostering of care shifts, ensuring all absences are satisfactorily planned for and covered.

To assist the NCM with the recruitment and to ensure all staff complete relevant inductions, training, competencies, observation assessments, supervisions and appraisals.

To promote a culture of teamwork, continuous improvement, staff retention and development.

To be responsible for the emergency calls and ensure appropriate action is taken.

In conjunction with the NCM you will provide out of hours support and advise staff and respond to emergencies.

Carry out walk about and CQC mock audits with the NCM

To keep accurate and confidential records.

Perform other duties as may from time to time be reasonably required.

Flexible approach to work to meet the needs of the service.

Key Skills/Experience:



Good general education with minimum GCSE in Maths and English or equivalent

Experience of working in caring environment as a Senior Carer/Team Leader, regulated by the Care Quality Commission (CQC).

Awareness of issues affecting older people and their families.

Self-motivated and proactive.

An excellent communicator.

Strong supervisory experience, including leading a team including coaching care and support staff, handling complaints and implementing service improvement.

Required to hold a level 3 qualification in Health & Social Care (QCF/NVQ).

IT competent Windows & Microsoft Packages.

Person Centred Care Planning approach.

General:



You are responsible for ensuring that you are aware of the Health and Safety policies and procedures for Nautilus Welfare Fund and that you take responsibility for your own health and safety whilst at work.

As we work with vulnerable adults, and come into contact with children, you will be able to recognise signs of or actual abuse and be aware of your responsibilities in relation to safeguarding.

You are responsible for your own personal development and will work with your line manager to ensure that you are adequately skilled to perform your job and participate in the annual learning and development programme.

Job Type: Full-time

Pay: 13.65 per hour

Expected hours: 35 - 40 per week
