Job Description

An exciting opportunity has arisen to be a part of the Lung Cancer Screening (LCS) programme in South East London, hosted by Guy's and St Thomas' Foundation Trust. The postholder will sit within the Lambeth Stop Smoking Service, but will work across all 6 boroughs of South East London (Bexley, Bromley, Greenwich, Lambeth, Lewisham and Southwark).



We are looking for an experienced Tobacco Dependence Specialist who has significant experience in community based settings, and can work highly independently.



You will be supporting the LCS programme working on a mobile unit around South East London with respiratory colleagues screening patients who are at risk of lung cancer. Your role will be to deliver specialist tailored tobacco dependence treatment to current smokers who want to stop smoking, including advising on tobacco dependence treatment such as vaping and/or nicotine replacement therapy and ensuring participants are supported to continue a full quit attempt.



As a brand-new role the postholder will be trying new ways of working, including developing and testing different work models to find the most effective method for delivering tobacco dependence treatment in this new setting.



Working across the various stop Smoking Services that cover the boroughs you will be required to learn several different systems for recording patient care, so strong IT skills and ability to learn new systems quickly is essential.



Duties and responsibilities will include:



Being an active team member of the SEL LCS programme, working in close liaison with other members of the multidisciplinary team



acting as a delivery lead for integrating tobacco dependence treatment into the LCS programme establishing systems and processes to facilitate this, ensuring effective tobacco dependence treatments within the LCS setting, in line with the standard protocol and quality assurance standards



setting up, delivering and managing onsite clinics, working collaboratively with the nursing team for the LCS programme



ensuring staff members of the LCS programme are trained in very brief advice (VBA) either through National Centre for Smoking Cessation and Training (NCSCT) online training or face-to-face training



being responsible for accurate and timely data entry onto the required system(s) including outcome data on the success of the quit attempt by following up directly with participants, and assisting in the preparation and submission of reports



working closely with GSTT's Tobacco Dependence Treatment Team and collaboratively with providers of local stop smoking services, ensuring optimal support pathways for smokers



contributing to the ongoing development, monitoring and evaluation of the tobacco dependence treatment offer within the LCS setting, improving service delivery and sharing good practice with other health professionals

To assess tobacco dependence, devise a treatment plan and ensure tobacco dependence care is given



To uphold and actively support the Trust's smoke-free status

Ensure targets are met



To manage a caseload of clients independently and prioritise service user needs



Develop expertise in effective treatment programmes for people at risk of, or living with lung cancer and/or a range of smoking related health conditions



Plan and deliver a training programme to promote a systematic, proactive approach to tobacco dependence across the service.

To utilise a range of different approaches to raise awareness of tobacco dependence services and to use all possible opportunities to generate referrals for patients



To promote and facilitate the development of seamless care pathways and referral systems between the screening team and professionals in acute settings, community health services and primary care in each borough where appropriate



To support the integration of the tobacco dependence service into broader health development initiatives



To contribute to the ongoing development, delivery, monitoring and evaluation of the tobacco dependence treatment service
