Juventas is looking for a multi-skilled, enthusiastic and dedicated

Transitions Support Advisor (full time, permanent)

to provide support, guidance and key working to young people who may have limited English language skills on arrival, helping them navigate living successfully in a new country and preparing them for the next stage of their lives.





The role





Juventas cares for looked-after children in residential and semi-independent supported accommodation so they can live a positive and meaningful life as they approach adulthood.





As Transitions Support Advisor your work will include:


+ Ensuring all UASC are supported to attend legal appointments with regards to their immigration status; + Acting as keyworker to young people, ensuring that all relevant young people's records are concise and kept up to date in line with Juventas policies;
+ Working to a Service Rota to help ensure each young person received 10 hours support per week
+ Leading on effective multi-agency liaison with Social Workers, Solicitors, The Home Office, Personal Advisers, GP's, Looked After Children Nurses, Education staff and voluntary agencies, parents and carers;
+ Assisting in supporting the referral and move-on processes and responding appropriately to requests for the service, within the context of Juventas Service policies;
+ Organising and supporting structured activities consistent with young people gaining Independent Living skills;
+ Organising leisure activities as required in consultation with young people;
+ Responding to and recording any safeguarding issues in relation to young people in line with Juventas policies;
+ Participating in and contribute to their own regular Supervision sessions, annual appraisal and service performance targets;
+ Ensuring their communications contribute to an effective staff team;
+ Assisting in ensuring that building, plant, equipment and furniture is well maintained, cleaned and cared for with appropriate input from young people;
+ Liaising in a professional manner at all times with neighbours and the local community;
+ Ensuring a timely response to any young people's concerns;
+ Participating in and contributing to regular staff meetings;
+ Ensuring that young people's Risk Assessments are kept up to date;
+ Setting high standards in respect to their own professional conduct and development.

The ideal candidate





You'll have:


A UK driving licence, business insurance (car) and the ability to reliably commute in the community. A strong understanding of safeguarding responsibilities An NVQ Level 3 in Health and Social Care (for the right candidate we are willing to offer training pathways to this qualification) A caring and empathetic nature, someone who wants to make a real difference!
