About you



The role of a Care Home and Home Care 'Trusted Assessor' will suit someone who has had Care Home / Home Care experience and has been involved in completing assessments for clients who require care packages within the Care Home / Home Care environment.

This position has been developed to bridge the gap between Care Home and Home Care and NHS teams to support older adults who have been assessed as requiring care packages within their own homes as well as completing assessments for Care Homes.

Your advice, guidance and support are crucial to your success in the role; therefore, you must have experience of working within the Care Home / Home Care environment. You will be required to go out and meet Care Home and Home Care managers and build relationships with the managers and providers to really understand the type of services offered by each provider. You will be liaising with Discharge Practitioners and the Hospital Social Work team to assess clients on behalf of the Care Home and Home Care providers in the acute setting for care packages in a care environment.

About the role



Finding the right package of care for family members isn't easy and helping someone to make the transition from an acute hospital setting to a care environment comes with its own unique challenges.

You will report to the Care Network Project Support Officer of the Bedfordshire Care Group LTD, but on a day-to-day basis, you will develop relationships with a wide range of services and individuals, such as hospital Discharge Practitioners, Social Workers, Ward staff and the providers of the Care Home and Home care services. You will be based across 3 sites Bedford, Luton & Dunstable and Milton Keynes.

We need an individual that is pragmatic and continually looking to create working partnerships. You need to be credible and can work with Care Homes and Home care providers to improve access to services, and to ensure effective and quality outcomes for clients is critical. You will need to be able to work autonomously within a pressurised environment and capable of problem solving as an individual as some on call work at weekends is required.

Salary: 28,000 to 34,500 (dependent on experience)



Hours: 40 hours per week, including weekends and on call when working if necessary across both acute sites of Bedford, Luton and Milton Keynes working on an 8-4pm rota.



Contract: 12-month Fixed Term (option to extend)



Essential qualifications/experience:



At least 5 years' experience preferably within a Care Home/ Home Care Environment with some leadership experience.

Level 3 Health and Social care award

You will have experience in completing assessments to a high standard for vulnerable adults requiring care packages within Care Homes / Home Care environments

Full UK Driving Licence

Competent IT skills including data reporting in Excel.

Job Type: Fixed term contract
Contract length: 12 months

Pay: 28,000.00-34,500.00 per year
